About the role:

The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)

is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values

and ensures that those values guide all decisions. Creating a caring environment for patients and families ensures

continuity of care and fosters a learning environment. Applicants are required to have a demonstrated knowledge,

understanding and commitment to this care philosophy.

Reporting to the Forensic Mental Health Program Manager, the Forensic Social Work role is to provide social work

services to the inpatient, outpatient and Northwestern Ontario Sexual Behaviours Program that would include

psychosocial assessments, treatment, counseling and after care planning.

Applicants must have the following qualifications:

* Honors Bachelor of Social Work degree

* Registered with the Ontario College of Social Workers and Social Service Workers

* Experience working with patients in a mental health setting

* Minimum of 3 years Forensic mental health experience

* Highly developed assessment skills

* Experience consulting and collaborating with members of the clinical team

* Excellent written and verbal communication skills, including the ability to formulate and type reports in a timely

fashion

* Self directed and able to work independently

* Thorough knowledge of theory and practice of professional Social Work, treatment and practices that would be

applied in a mental health or related setting

* Extensive and demonstrated knowledge of community resources

* Knowledge and experience with principles of Psychosocial Rehabilitation or a similar recover based model of care

* Mental Health Act and its application

* Understanding of the Health Care Consent Act, the Substitute Decision Act and Ontario Review Board

Salary Range: $39.160 to 50.770
