About the role

This is a temporary contract for up to six (6) Months

Clinical pharmacy in an acute care hospital is where your expertise finds a high level of application, within an interdisciplinary care team, at the bedside, supporting complex patient care. As a Clinical Pharmacist at Mount Sinai Hospital, you will practice within a clinical area that offers both focus and the kind of case complexity that sharpens expertise over time. This precision ensures medications are optimized and patients receive the benefit of your informed judgment.

Effective collaboration in acute care requires thoughtful engagement. You communicate across disciplines with clarity, translating complex drug therapy insights into recommendations that prescribers, nurses, and other health disciplines can act on. Whether contributing on rounds, responding to a real-time consult, or co-developing a medication treatment plan, you ensure that drug therapy considerations are embedded in care decisions from the start, helping the team move towards a clear, informed course of action. In a hospital setting where case complexity is the norm, this approach to collaborative engagement helps ensure patients benefit from clinical pharmacy expertise throughout their journey with us.

Are you skilled at translating complex medication information into clear, practical language that patients and families can truly understand and follow? During discharge conversations or therapy changes, you take the time to explain not just what is changing, but why, giving patients the clarity they need to feel confident in their care. You anticipate the questions patients and families may not yet know to ask, ensuring nothing is left unclear and that every patient leaves better informed and better prepared than when they arrived.

About this team:
The Pharmacy Department at Mount Sinai Hospital delivers comprehensive clinical pharmacy services across a broad and specialized acute care environment. Our team of over 50 pharmacists is diverse, dynamic, and composed of specialists across a wide range of clinical areas, as well as drug information, antimicrobial stewardship, research, and clinical informatics fostering a highly collaborative and supportive practice environment. Our clinical pharmacists are fully integrated into interprofessional teams serving adult, older adult, and neonatal populations across medical and surgical units, intensive care, emergency care, psychiatry, and oncology. We develop patient-specific pharmacotherapy care plans and collaborate closely with our teams to provide evidence-based recommendations to prescribers and other health care professionals.

We are committed to continuous learning and provide opportunities for professional development and growth, and provide scheduling and financial support for relevant Continuing Education opportunities, including conferences, workshops, and Board of Pharmacy Specialties certification. As affiliates of the University of Toronto Leslie Dan Faculty of Pharmacy and the University of Waterloo, School of Pharmacy, we are actively involved in training the next generation of pharmacists, including opportunities to provide experiential preceptorship to pharmacy learners and participate in academic teaching.

In this role you will:

  • Provide expert recommendations on drug selection and dosing to optimize medications while minimizing potential risks
  • Design and implement individualized medication treatment plans in collaboration with the interprofessional team
  • Perform comprehensive medication reconciliation to ensure patients receive their required home medications throughout their hospital stay, and are prescribed appropriate medications upon discharge
  • Conduct drug therapy monitoring alongside order entry and drug distribution to ensure the accuracy and clinical appropriateness of medication therapy
  • Participate in clinical rounds to provide real-time drug information and therapeutic guidance to the care team
  • Deliver patient education, including explaining medication changes and addressing therapeutic questions to ensure a clear understanding of the treatment plan
  • Provide discharge counselling to ensure patients and care givers understand medication changes, and ensure seamless transition to the community
  • Support central dispensary operations as needed
  • Contribute to the teaching and training of various levels of pharmacy and interdisciplinary learners
  • Participate in relevant interprofessional hospital committees and quality improvement initiatives
  • Perform other duties consistent with the job classification as required

Job Requirements

Job Requirements:
Mandatory

  • Successful completion of a Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited educational institution

  • Active (or eligible for) registration with the Ontario College of Pharmacists (OCP)

Preferred

  • Residency training or relevant inpatient experience an asset
  • 3 years of experience in pharmacy

  • Demonstrated, excellent time management and organizational skills

  • Strong problem solving and critical thinking skills

  • Demonstrated satisfactory work performance and attendance record
