About the role

Title: Occupational Therapist

Reports To: Program Manager Location: Mississauga Salary: $70,000 -$95,000/ Annually

Position Summary

The Occupational Therapist will provide community-based support to youth and young adults experiencing complex mental health, addiction, and housing-related challenges. Working within a multidisciplinary team, the Occupational Therapist will help clients develop the skills, strategies, and supports needed to live independently and achieve their personal recovery goals.

This role combines clinical assessment, rehabilitation, advocacy, and community outreach, supporting individuals in their homes and throughout the community. The successful candidate will build meaningful therapeutic relationships, collaborate with caregivers and service providers, and contribute to a recovery-oriented model of care.

Key Responsibilities

Assess, develop, and monitor functional skills related to daily living, including budgeting, household management, personal care, community participation, and leisure activities. Identify barriers impacting independence and develop individualized treatment and recovery plans. Provide occupational therapy interventions that support clients in achieving meaningful personal, social, educational, and vocational goals. Deliver individual support, counselling, coaching, and psychoeducation appropriate to client needs. Support individuals experiencing mental health challenges, substance use concerns, and housing instability. Conduct risk assessments and participate in crisis planning and intervention when required. Work collaboratively with case managers, psychiatrists, healthcare professionals, family members, and community partners to coordinate care. Facilitate transitions between community, healthcare, and social service systems. Assist clients in developing and maintaining community supports and social connections. Maintain accurate clinical documentation and client records using the organization's electronic medical record system. Participate in case conferences, team meetings, and ongoing program development initiatives. Adhere to all organizational health and safety policies and procedures. Perform other related duties as assigned.

Qualifications

Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario. Experience supporting individuals with mental health, addiction, or psychosocial rehabilitation needs. Knowledge of Assertive Community Treatment (ACT) standards and community-based service delivery models is required. Understanding of homelessness, housing instability, substance use, and recovery-oriented practices. Experience working with youth and young adults is considered an asset. Strong assessment, treatment planning, crisis intervention, and risk management skills. Demonstrated ability to work independently while managing a community-based caseload. Excellent communication, relationship-building, and interdisciplinary collaboration skills. Proficiency with electronic documentation systems and Microsoft Office applications. Valid Ontario driver's license, reliable vehicle, and appropriate insurance coverage.

Working Environment

Community-based position serving clients throughout Peel Region. Typical caseload involves visiting multiple client residences and community locations throughout the day. Mileage reimbursement provided for work-related travel. Company-issued mobile phone provided. Comprehensive onboarding and safety training provided, including crisis prevention and intervention, first aid, and naloxone training.

Working Conditions and Physical Requirements

Standard schedule: Monday to Friday, 8:30 AM–4:30 PM, with flexibility required based on client needs Majority of work is completed in community settings, including client homes Moderate level of physical activity required Ability to sit or stand for extended periods Regular use of computers and telephone communication Ability to lift up to 20 lbs (e.g., documents or materials) Frequent travel to client locations and community sites as required
