Position Purpose:
Reporting to the Hospital to Home Manager, the H2H Coordinator is responsible for improving patient flow metrics. Support patient as they safely transition home following a hospital stay, continue communication with patients and families through program length. Coordinators identify appropriate patients, support admission avoidance and earlier discharge, onboard patients to the H2H model, and maintain communication between hospital teams and SPO’s. Support quality improvement and performance monitoring aligned with Ontario Health H2H targets.
Duties and Responsibilities:
- Provides coordination, planning, and other levels of support for patients that will be transition home from hospital through Hospital 2 Home program or Ontario Health @ Home.
- Collaborates with Service Provider Organization to manage and make adjustments to patient care plans that are currently enrolled in H2H program.
- Continues ongoing communication with patients and families throughout the duration on the H2H program.
- Facilitates the management of flow and onboarding on H2H by ensuring smooth intra/Interdepartmental communication.
- Collaborates with the multidisciplinary teams for admission avoidance and facilitating discharge on the H2H program or Ontario Health at Home.
- Commutes to other organizations within the region to complete assessments and facilitate onboarding onto the H2H program.
- Tracks and manages key metrics responsible for reporting and program success.
- Participation in the ongoing professional development of self and others.
- Ensures compliance and support for staff for clinical quality indicators.
- Complies with the Ontario Health directives
- Identifies opportunities to improve communication processes and patient experiences, sharing suggestions with the team.
- Participates in quality improvement projects in collaboration with the Utilization Team
- Assists with other program priority initiatives or projects in collaboration with the leadership team and other programs as needed.
- Other duties as assigned.
- Able to Flex hours as the program requires
Qualifications:

Completion of a recognized Health Care discipline Diploma or Degree program as deemed appropriate by the program.

3-5 years of direct work experience in an acute care hospital and/or project management/event coordination preferred.

Demonstrated success in project delivery and execution of project management methods.

French Language proficiency an asset.

Providing Acute Care Services with 650 plus beds and an operating budget of half of a billion dollars, Windsor Regional Hospital is one of the largest community based non-academic hospitals in the Province of Ontario.
