About the role:
The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)
is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values
and ensures that those values guide all decisions. Creating a caring environment for patients and families ensures
continuity of care and fosters a learning environment. Applicants are required to have a demonstrated knowledge,
understanding and commitment to this care philosophy.
Under the direction of the Manager North West Regional Renal Services, the Clinical Dietitian is responsible for
nutrition support to patients and families of Thunder Bay Regional Health Sciences Centre Renal Services.
Qualifications:
* Must be licensed Dietitian in the province of Ontario
* Experience in the care of patients living with kidney disease including those on dialysis
* Sound working knowledge of the nutritional needs of patients living with kidney disease in the various treatment
modalities
* Experience in nutrition support including enteral and parenteral nutrition
* Proven organizational, interpersonal and counseling skills.
* Clinical research skills.
* Effective oral and written communication skills.
* Experience with the Nutrition Care Process & Standardized Language.
* Experience working in an interdisciplinary team.
* Self motivated and enthusiastic.
* Minimum two years recent experience working with Patients and Families in both inpatient and outpatient settings.
Skill / Competencies
* Assessment of nutritional needs, developed and implemented care plans, evaluated nutritional outcomes
* Experience with the Nutrition Care Process & Standardized Language
* Develop and implement Patient and Family Centered Care education collaborating with the team that manages
plans based on Patient's goals
* Knowledge of chronic disease prevention and management
* Demonstrated critical thinking, problem solving and conflict resolution skills
* Strong interpersonal and communication skills
* Advanced Computer Skills
* Manage a demanding caseload
* Canadian Diabetes Education Certification is an asset
Salary Range: $41.680 to 53.280
The Health Sciences Centre strives to ensure the safety and security of the patients, visitors, employees and assets
