Reporting to the Director of Care, the RAI-MDS Coordinator is responsible for supporting the long-term care home in the management and sustainability of RAI-MDS processes to ensure the validity of clinical and quality outcomes that support the provision of care and services, in accordance with best practice, interRAI and legislative requirements.

RESPONSIBILITIES

  • Coordinate and monitor RAI-MDS processes within the home according to organizational policies and procedures, legislative requirements, standards of practice and inter-RAI requirements (CIHI).
  • Develop and maintain a current Resident assessment schedule and ensure it is accessible to all RAI users.
  • Maintain efficient processes for data collection to ensure a complete and accurate assessment that supports an individualized and comprehensive Plan of Care.
  • Facilitate RAI-MDS and application training (Point Click Care, Point of Care), provide ongoing leadership and mentoring assisting with knowledge application to support data accuracy and proficiency of all users.
  • Implement processes that support identifying gaps/discrepancies and provide ongoing education to reinforce knowledge, promote capacity and enhance staff competency.
  • Monitor RAI-MDS assessments, resident assessment protocols/clinical assessment protocols and care plan reviews for completion in accordance with required timelines.
  • Monitor, analyze and evaluate generated outcomes to support continuous quality improvement.
  • Generate RAI-MDS output reports and in collaboration with the interdisciplinary team, facilitate their use in clinical and operational decision-making.
  • Maintain ongoing review/audits of MDS assessment data elements and all resource utilization groups for accuracy and optimization potential prior to quarterly submissions.
  • Manages RAI-MDS submissions within required timelines, review CIHI reports and MDS corrections and resubmissions when necessary.
  • Manage and support all Relias LMS users in the home and ensure successful completion of the annual competency evaluations.
  • Perform other duties as assigned.

Internal applicants will be considered before external candidates.

REQUIRED EDUCATION, CREDENTIALS AND EXPERIENCE

  • Current registration with provincial College of Nurses as a Registered Practical Nurse (RPN).
  • A minimum of three (3) years clinical, project leadership or management experience
  • Demonstrated interest in the RAI Coordinator role and MDS program.
  • Minimal 1-year experience in the RAI Coordinator role (preferred).
  • Current knowledge of RAI-MDS 2.0 and experience in the use of Point Click Care system and Point of Care is an asset.
  • Thorough knowledge of LTC legislation, program standards, including documentation.
  • Demonstrated success leading new initiatives.
  • Effective inter-departmental communication and leadership skills are necessary.
  • Demonstration of well-developed oral and written communication skills.
  • Knowledge and experience with adult education strategies and facilitation.
  • Previous experience presenting to groups.
  • Well-organized, with excellent time management skills.
  • The ability to work with and relate to interdisciplinary team members and other internal stakeholders.
  • Strong and effective problem-solving abilities.
  • Proficient computer skills.
  • Current (within 12 months) Vulnerable Position/Sector Screening (VPS).
  • Compassion, professionalism, and respect.
  • Ability to read, write and communicate effectively in English.
