A DAY IN THE LIFE OF AN ABI INTAKE COORDINATOR

Reporting to the Director of Client Services, the Intake Coordinator will be responsible for a client centered approach, working with a multi-disciplinary team. Their responsibility includes:

  • Completing the intake process for ABI clients referred to Traverse Independence utilizing a common screening/intake tool, and accessing of external records and information as appropriate
  • Developing a comprehensive intake report to be presented to intake team and/or at Admissions and Transitions Committee meetings
  • Liaising with community partners such as educational facilities, regulated professionals and other service providers, families and caregivers.
  • Participating in resolution meetings for ABI clients with unique needs which may include a multi-agency or cross sectoral approach
  • Ensuring all data is entered and collected as required and maintaining global data as required
  • Ongoing and regular contact with clients on the waiting list, families, community members and external partners.

Personal interaction, assessment skills, presentations and liaising with key community organizations will be integral to the success of this position along with collection, tabulation and analysis of data. As a part of the Coordinator team, the ABI Intake Coordinator may be responsible for other duties as assigned.

QUALIFICATIONS

Education:

2 year post secondary diploma in social services, health care, or other related discipline.

Experience:

A minimum of 2 years experience with ABI, experience supervising or leading a team would be useful but not necessary.

Knowledge/Skills:

  • Excellent communication, interpersonal, organizational, time management and presentation skills
  • Proven ability to meet deadlines, handle multiple tasks, function well under pressure and work effectively as a team member all required
  • Familiarity with current legislation, health and safety requirements, and Ministry of Health requirements
  • Comprehensive knowledge of computer hardware and software and previous experience with GoldCare a definite asset
  • Must have valid Class G driver's license and reliable vehicle. Travel will be required
