Job Description: The Assessor Coordinator reports directly to the Home Care Manager. The Assessor Coordinator is responsible for assessment/reassessment services with ongoing client care planning and management. The Assessor Coordinator is accountable for establishing and maintaining positive liaison between the Health Centre and Home Based Services. The Assessor Coordinator is accountable for screening, referring, reviewing, planning and facilitating safe care for individuals. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Experience
- One (1) year related experience in health care or social sciences.
Education
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- Bachelor of Physical Therapy is acceptable
- Bachelor of Occupational Therapy is acceptable
- Psychiatric Nursing diploma is acceptable
- Practical Nursing diploma with 4 years experience is acceptable
- Bachelor of Social Work is acceptable
- Bachelor of Psychiatric Nursing is acceptable
- Bachelor of Psychology is acceptable
- Nursing diploma is acceptable

- Foam Lake is located approximately 92 km west of Yorkton
- Preference will be given to candidates with community experience.
- Scheduled hours of work may be reduced during the rotation stipulated when a statutory holiday is recognized during that period.
