Position Summary The Patient Flow Facilitator is a clinical and operational leader responsible for supporting safe, timely, and efficient patient movement across Trillium Health Partners. Working within the Trillium Operations Centre (TOC), the role partners closely with interdisciplinary teams to proactively identify and remove barriers to discharge, optimize inpatient capacity, and support appropriate patient placement, transfer, and discharge decisions across programs and sites.
Using advanced clinical judgment, system awareness, and real-time decision-making, the Patient Flow Facilitator evaluates new and admitted patients through chart review, consultation, and relevant data sources to support patient centred flow decisions. This position facilitates timely discharge, alleviates access and capacity challenges, and functions as a crucial clinical resource and escalation point to ensure patients receive care in the most appropriate care setting
Responsibilities
- Proactively monitor patients approaching clinical stability and identify opportunities to support same day or next day discharge
- Expedite outstanding diagnostics, treatments, and consults impacting discharge readiness
- Escalate delays and risks to unit leadership and appropriate stakeholders
- Identify opportunities to create inpatient capacity (e.g., cohorting, utilization of over census beds)
- Collaborate closely with the Patient Flow Coordinator to implement site-specific flow plans
- Attend bed meetings to support situational awareness, risk mitigation, and capacity planning
- Utilize flow and capacity management tools (including Epic) to support standard work and decision-making
- Provide guidance and direction to clinical leaders, physicians, and interdisciplinary care teams
- Maintain timely, clear, and effective communication with key internal stakeholders
- Act as a subject matter expert and escalation resource for patient flow issues
- Contribute to patient flow improvement initiatives, including the development and refinement of policies, procedures, and standard work
Key Qualifications
- Undergraduate degree (or equivalent)
- Regulate health care professional (nursing or health discipline)
- Minimum five (5) years recent clinical experience in an acute care hospital setting
- Experience in acute medicine and/or discharge planning considered an asset
- Patient Flow or capacity management experience considered an asset
- Familiarity with Epic preferred
- Demonstrated leadership skills with the ability to influence, collaborate, and escalate appropriately
- Strong clinical judgment and system level awareness
- Excellent written and verbal communication skills
- Strong organizational and time management skills with attention to detail
- Self-directed, adaptable, and able to thrive in a fastpacked, dynamic environment
- Demonstrated commitment to patient centred care
- Proven ability to build positive, collaborative relationships with physicians, clinical leaders, and interdisciplinary teams
- Positive work attendance is essential
