About the role
We are seeking experienced Nurse Practitioners (NPs) to conduct comprehensive in-home assessments and complete required clinical documentation, including insurance related documentation in accordance with Ontario regulations.
This role is ideal for Nurse Practitioners who are comfortable working independently, conducting structured assessments, and preparing detailed clinical reports for insurance and rehabilitation purposes.
Key Responsibilities
- Conduct comprehensive in-home functional, nursing, safety, and ergonomic assessments while developing clear clinical recommendations based on findings.
- Complete, sign, and submit Form 1 and OCF documentation in compliance with Ontario regulatory and insurance standards, ensuring accuracy and timeliness.
- Prepare detailed, objective assessment reports and communicate findings effectively with insurers, legal representatives, and interdisciplinary teams as required.
- Maintain professional documentation standards while working independently and managing a flexible caseload schedule.
Requirements
- Masters degree in Nursing (Nurse Practitioner stream) with active registration as a Nurse Practitioner in Ontario and authorization to complete and sign insurance treatment plans and required documentation as the health service provider.
- Demonstrated experience conducting independent clinical assessments with strong report-writing and analytical skills.
- Professional liability insurance, reliable transportation for in-home visits, and the ability to work autonomously in a contractor-based model.
- Experience within auto insurance, medico-legal, rehabilitation, or community-based assessment environments.
