Social Worker –U of T Health & Wellness Partnership(Full Time – Permanent)

The Role and Opportunity CMHA Toronto, in partnership with the University of Toronto Health & Wellness Centre, is seeking a skilled and compassionate Social Worker to support post-secondary students both on campus and in the community. This role is anchored within CMHA Toronto’s MOD program and offers the opportunity to deliver client‑centered, trauma‑informed mental health support within an innovative campus–community collaborative partnership, working alongside an interdisciplinary team dedicated to strengthening student mental health and positively influencing students’ wellbeing, resilience, recovery journeys, and continued engagement in their studies.

The successful candidate will work directly with U of T students presenting with a range of mental health concerns, providing assessment, intervention, referral, and care coordination within a collaborative campus–community partnership. While employed and supervised through CMHA Toronto, the Social Worker will deliver services in close collaboration with U of T Health & Wellness Centre clinicians and staff, supporting students both on campus and through community-based outreach, ensuring seamless continuity of care.

Salary: $63,879.19 – $69,994

Responsibilities

  • Provide care within a collaborative partnership with U of T Health & Wellness Centre, supporting students with identified mental health and social determinant of health needs
  • Develop a therapeutic alliance with students referred to the program, supporting their mental health recovery, facilitating connection to community resources and social services that address social determinants of health, and promoting their continued engagement in studies at the University of Toronto.
  • Provide psychoeducation about medications, symptoms, and mental health concerns.
  • Monitor recovery progress, complete assessments, and engage in collaborative goal planning.
  • Assess mental status and monitor physical health needs.
  • Assess client needs and link to appropriate resources using a holistic lens, including supports related to the social determinants of health.
  • Work from a recovery-based, trauma-informed, client-centered perspective.
  • Collaborate on service provision with CMHA and U of T Health & Wellness Centre team members, as well as community partners and resources.
  • Collect data and maintain documentation to support partnership goals and quality improvement initiatives.

Experience & Education Required

  • Master's degree in social work and registered in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Minimum of three (3) years of mental health experience; community-based experience and experience with students/young adults is a strong asset.
  • Excellent skills in mental health assessment, teaching, clinical decision-making, case management, and effective crisis intervention.
  • Ability to deliver care within a collaborative partnership supporting students working closely with the U of T Health & Wellness Centre.
  • Demonstrated ability to intervene effectively in clinical situations involving youth and adults experiencing mental illness.
  • Ability to demonstrate a non-homophobic, anti-racist, anti-oppressive approach and work effectively with diverse communities.
  • Strong communication, active listening, and relationship-building skills.
  • Fluency in a second language is considered an asset.
  • Commitment to ongoing professional development and incorporation of evidence-based practices.
  • Proficiency using electronic health records and comfort with data literacy to support program outcome measurement.
  • Access to a car and a valid driver’s license is required.
