The time where there is a suspected cancer diagnosis is often a very stressful time for patients and caregivers,

Many patients see multiple healthcare providers and have many tests before they receive a diagnosis, this can be a long difficult process for patients and caregivers. Diagnostic Assessment Programs (DAPs) help patients and providers navigate this process. Once referred into a DAP, patients have access the interprofessional care team to assist in this journey, including a patient navigator who is the patient's main contact and coordinates all testing for the patient throughout the diagnostic journey.

Summary of Duties

Reporting to the Director of Operations, the Odette Cancer Program is seeking a Registered Nurse to work collaboratively with key stakeholders to work within the DAP program, focussing on the GI population. Duties include providing navigation, coordination and support to patients referred to the DAP and during their journey from suspicion to diagnosis to treatment. Responsibilities include establishing pathways/directives, triaging referrals, completing a full telephone assessment, identifying potential complications and booking necessary diagnostic/staging tests. This RN will be responsible for following up on these tests and making sure patients have follow-up appointments and tests booked. They will coordinate with physicians closely to monitor the progress and wait times for each patient. They will participate in multidisciplinary case conference rounds as well as attend any courses or conferences applicable.

The RN is responsible for the collection of data as well as well as have a working knowledge of the Colorectal DAP, administrative Lung DAP, and Allogenic Transplant Program Protocols for coverage as required.

Qualifications/Skills:

  • Current registration with College of Nurses of Ontario

  • Baccalaureate degree in nursing required

  • Experience in ambulatory care, critical care, emergency nursing preferred

  • Ability to work independently

  • CPR –Basic provider certification required

  • Excellent interpersonal, oral and written communications skills

  • Proficient in Microsoft products (Excel, Outlook, Powerpoint, Word)

  • Experience in following medical directives

  • Ability to facilitate in creating and leading meetings
