Job Summary & Requirements Clinical Pharmacist – Quality Lead Reporting to the Manager, Clinical Pharmacy Services, the Clinical Pharmacist-Quality Lead is an integral member of the Pharmacy Team. The Clinical Pharmacist – Quality Lead role is divided between front-line clinical pharmacy practice and leading quality initiatives across the organization. This position will work in collaboration with a variety of departments across the hospital including, but not limited to, professional practice, professional staff, quality and risk, among others.

The successful candidate will primarily provide direct patient care as a clinical pharmacist, with the remaining time dedicated to serving as a key liaison between the Pharmacy Department and other departments engaged in quality improvement initiatives.

As the Quality Lead, this role will contribute to the development and maintenance of safe medication practices, including parenteral drug monographs, smart pump library updates, and other interdisciplinary quality initiatives that support optimal medication use across the organization.

As a clinical pharmacist, the Clinical Pharmacist – Quality Lead, is also accountable and responsible for completing patient medication assessments, ensuring optimal medication therapy and minimizing potential for adverse events when supporting direct patient care. This role is expected to provide accurate medication information to the interdisciplinary team. The pharmacist promotes safe and appropriate use of medications in a multidisciplinary setting to minimize risk and optimize health care resources as they pertain to medications.

Responsibilities: Clinical Practice

  • Complete patient medication assessments
  • Ensure optimal medication therapy and minimization of potential adverse events.
  • Review of lab information to ensure appropriate drug therapy.
  • Participate in patient rounds in clinical area of practice.
  • Participate in drug distribution, including accurate order entry of physician orders.
  • Provide pharmacists and other healthcare professionals with acquiring relevant drug information.
  • Review and provide recommendations on various document submissions relating to clinical area of coverage.
  • Work collaboratively with interdisciplinary team
  • Complete appropriate medication safety audits to align with Accreditation Canada standards.
  • Participate in the education and mentoring of new staff
  • Precepting students and pharmacy residents, with minimum commitment of mentoring one pharmacy learner per year
  • Participate in the education and ongoing learning of nursing staff.

Quality Lead

  • Act as the pharmacy representative on committees, projects, and working groups collaborating with interdisciplinary departments and groups.

  • Collaborate with Nursing Professional Practice and Clinical Educators on:

  • Development, review, and standardization of parenteral drug monographs.

  • Review and updates of smart infusion pump drug libraries.

  • Contribute to review of medication administration policies and guidelines.

  • Support implementation and communication of practice related changes, as result of project or committee work, to front line staff.

Education and Licensing:

  • Registered Pharmacist, in good standing with the Ontario College of Pharmacists (Part A)
  • Bachelor of Science or PharmD degree in Pharmacy

Experience:

  • Post-graduate Pharmacy Residency Program (ACPR), an asset
  • Minimum two (2) years of experience in a hospital pharmacy setting, preferred

Competencies:

  • Knowledge of current evidence-based, best practice pharmacotherapeutics and their application.
  • Practice continuing professional development as required by Regulated Health Professions Act.
  • Knowledge of Parenteral Monographs and Infusion Pump safety parameters.
  • General knowledge of appropriate dosages, delivery techniques, formulations and routes of administration of all medications.
  • Knowledge & interpretation of laboratory and clinical parameters and their application to patient assessment & monitoring.
  • Knowledge & utilization of credible electronic & print form medical information resources
  • Knowledge of third-party reimbursement strategies
  • Knowledge of health promotion & disease prevention
  • Knowledge of complementary medicines & alternative therapies
  • General knowledge of aseptic technique & extemporaneous preparation of pharmaceuticals
  • Knowledge of multiculturalism and diversity
  • Knowledge of Microsoft applications
  • Understanding and interpretation of research methodology and outcomes & critical appraisal of study design
  • Excellent time management and organizational skills
  • Ability to function as an independent practitioner and a strong collaborator in a group setting
  • Sound problem solving, communications (oral and written), conflict resolution and decision-making skills
  • Highly motivated team player with the ability to collaborate across disciplines/departments/programs
  • Ability to perform multiple tasks and work under pressure
  • Contribute to a strong teamwork environment
