About the role
Company Description MEPEC (Monteregie East Partnership for the English-speaking Community) is the 22nd Network and Partnership Initiative (NPI) in Québec, dedicated to improving access to health and social services for minority English-speaking communities in the region. Funded by Health Canada and the Enhancing Regional Community Capacity (ERCC) program, MEPEC works closely with the Community Health and Social Service Network (CHSSN) and other government partners. The organization aims to strengthen regional community capacity and better understand the needs and priorities of English-speaking residents. MEPEC’s mission is to serve the interests of the English-speaking population in Monteregie East by connecting, collaborating, communicating, and facilitating with citizens and community partners. Team members contribute directly to more inclusive, accessible services for local communities. Role Description The Community Patient Navigator is a full-time hybrid role based in Beloeil, QC, with a mix of on-site work and some work-from-home flexibility. The person in this role supports English-speaking community members in understanding, accessing, and navigating health and social services, including appointments, referrals, and community programs. Daily activities include providing information and guidance, assisting with forms and documentation, coordinating with healthcare and social service providers, and following up to ensure continuity of care. The Community Patient Navigator also collaborates with community organizations, participates in outreach activities, and helps identify gaps or barriers faced by patients, sharing insights with MEPEC’s team to improve services. This role involves maintaining accurate records, respecting confidentiality, and promoting person-centered, culturally sensitive support.
Qualifications
Strong communication and interpersonal skills, including the ability to listen actively, explain complex information clearly, and build trust with diverse community members. Fluency in English and good working knowledge of French, with cultural awareness and sensitivity to the needs of minority language communities. Experience working or volunteering in community health, social services, or community organizations, ideally with knowledge of the Québec health and social service system. Organizational and coordination skills, including managing appointments, documentation, and follow-up, while maintaining confidentiality and accurate records. Ability to work independently and collaboratively in a small team, with flexibility to support outreach activities and occasional events in the community. Proficiency with basic office software, email, and online communication tools, and comfort using databases or tracking tools for client information. Post-secondary education in social work, community services, health sciences, or a related field, or equivalent combination of education and relevant experience. Commitment to equity, inclusion, and advocacy for improving access to services for English-speaking and other marginalized communities.

MEPEC also receives funding for the Enhancing Regional Community Capacity (ERCC) program provided by the Secrétariat aux relations avec les Québécois d’expression anglaise. Also managed by CHSSN, this program will help MEPEC better service it’s ESC and increase its understanding so as to better respond to the ESC’s needs and priorities in relation to government, regional and community stakeholders.
