About the role

Job Purpose The HELP Healthcare Navigator will be responsible for directly supporting HELP Navigators to meet the medical needs of their clients. Duties include, but are not limited to the following: Support with Client Centered Medical-Related Needs (70%)

  • Assist with determining the medical history and status of clients during the onboarding process.
  • Access appropriate databases and provide information pertaining to client’s lab work, medication profiles, pharmacy locations, healthcare number, hospital involvement, and medical program capacity.
  • Assist with the completion of detox and healthcare related referrals.
  • Attend medical appointments and advocate for the patient, if required.
  • Provide appointment reminders and complete follow up as needed.
  • Can facilitate specialist appointments.
  • Connect patient to primary care health provider.
  • Conduct minor wound care.
  • Provide medical and health related education to clients.
  • Offer medical related consultation.

Education, Experience And Requirements

  • LPN, RPN, or RN degree or diploma. A minimum of 3 years relevant experience working with complex populations, youth, and families would be an asset. It is expected that they maintain and are in good standing with any relevant professional bodies they may be members of.
  • Strong understanding and knowledge of healthcare-related community resources, and knowledge of issues faced by individuals experiencing homelessness, poverty, mental health conditions, and substance use.
  • Strong leadership, teamwork, and organizational skills, along with effective interpersonal and communication skills.
  • Experience working with and understanding the unique challenges that Indigenous and multicultural individuals and families face.
  • Risk management.
  • Partnership and stakeholder navigation.
  • Advocacy within healthcare systems.
  • Group facilitation.
  • Role and scope navigation.
  • Experience in mobile outreach or community-based settings.
  • Knowledge of trauma-informed care and harm reduction principles.
  • Proficiency in Cree or other Indigenous languages is an asset.
  • Passed an Enhanced Security Clearance.
  • Candidate has completed training in Suicide Intervention, Standard First Aid, Crisis Intervention, and De-escalation
  • valid Alberta driver’s license, Alberta registration, insurance, with the ability to operate a personal vehicle.
  • Criminal Record Check with Vulnerable Sector.

Working Conditions

Shelter will operate 24/7, which may require employees to work during evenings, weekends, and holidays. Shift schedules may vary, and flexibility in working hours may be necessary. While adhering to the Enoch Cree Nation Human Resources Policy & Procedure Manual.
