About the role

Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.

With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail.

We are currently seeking a Community Ambassador to join our Community Health team.

Terms: Full-time Permanent

Location: 300 Silver Star Blvd., Scarborough and community locations in the York Region and City of Toronto

Essential Duties and Responsibilities:

  • Working closely with an interprofessional team, the Community Ambassador will play an integral role of: engaging in culturally responsive engagement and outreach to individual community members and provide accurate and relevant information/education about the importance of preventive health care, cancer screening, vaccination, chronic disease prevention and management, mental health promotions, access to primary care, and other local population health challenges
  • Conduct culturally and linguistically responsive educational presentations individually and within groups in the communities
  • Engage community members in preventive care conversations and health action planning
  • Connect community members to the right services and programs that support the determinants of health (i.e. service navigation to relevant local community resources). E.g. help facilitate primary care, cancer screening referrals.
  • Conduct grassroot community outreach activities, i.e. in-person and virtual/social media
  • Participate in relevant committees and service networks internally/externally
  • Collect and track and help analyze statistics for trending and reporting and program evaluations
  • Actively participate in the development, implementation, and evaluation for this innovative model of health promotion and wellness clinics in the community
  • Other duties as assigned in support of operations

Qualifications & Job Requirement:

  • Post-secondary education in a relevant field (e.g. health promotion, human services) an asset. Appreciate a combination of lived/living and work experience relevant to the role
  • Experience with community-based outreach in an employment or volunteer capacity
  • Valid Ontario Driver's license and access to a vehicle
  • Demonstrated current expertise/experience in EDI and anti-racism practices, integrated primary care and management, chronic disease prevention and management, health promotion and wellness.
  • Fluency in a second language that reflects the communities that we serve
  • Strong understanding of demonstrated knowledge, skills and abilities in social services, community work, community resources, and understanding in equity-based population health approaches.
  • Ability to work independently and collaborate effectively in an interdisciplinary team environment.
  • A valid vulnerable sector police record check completed within one year
