Schedules are subject to change to meet departmental needs.
Reporting to the Supervisor, Developmental Services – Community Programs, the Passport Navigator provides intake, service coordination, advocacy, and Passport funding administration for adults with developmental disabilities and/or dual diagnosis. This supports individuals and families to effectively manage Passport funding, including budget development, eligible purchases, coordination of supports, and timely submission of claims through the PassportONE system. The Passport Coordinator also supports outreach, maintains accurate records, and works collaboratively with internal teams and external partners to ensure funding compliance, quality outcomes, and continuity of service.
Requirements
WAYS YOU CAN CONTRIBUTE
-Funding Coordination:
-
Collaborate with families and individuals to support the administration of their own direct funding (Passport) up to $35,000 per year.
-
Support individuals to use their funding effectively, including developing a funding budget, contacting and scheduling support workers, making required purchases, and completing and submitting all required forms through the PassportONE system.
-Intake/Needs Assessment:
- Completing intake and needs assessments with individuals entering service or whose needs have changed.
-Administrative Duties:
-
Ensure accurate and timely completion of all required forms and reports, in accordance with Springboard and/or funder requirements.
-
Maintain databases through My Community Hub.
-
Support the processing of invoices for individuals and families
-Quality Assurance:
-
Ensuring all individual outcomes are met in accordance with Quality Assurance Measures requirements, as outlined by the program, participants and funder.

Service Coordination:
-
Conduct needs and risk assessments, including identifying strengths and areas of development
-
Providing advocacy, referrals and coordination of supports.
-
Representing Springboard at community-based committees, networks, and tables as assigned
WHO YOU ARE
-
Post-Secondary Diploma in the social service field.
-
At least 2 (two) years of experience in coordination roles, including advocacy and coaching individuals with developmental disabilities and/or individuals with dual diagnosis;
-
Experience creating and implementing developmental goal plans, including Person Directed Planning.
-
Experience in budget administration including developing individual funding budgets, allocation of funds for services and support and completing purchasing and claims submissions.
-
Experience supporting, motivating, and advocating for adults with developmental disabilities and/or dual diagnosis, including outreach and development of community partnerships.
-
Strong knowledge of community resources including Community and Developmental Services sector.
-
Experience and knowledge of Passport and PassportONE system, including eligible expenses, submission processes, and administrative responsibilities.
-
Ability to accurately assess individual needs, develop plans, and identify achievable goals.
-
Ability to work with individuals, families, and supports while balancing multiple operational demands.
-
Proficient with computers and online case management systems (i.e. Nucleus Labs), Microsoft Applications (e.g. Word, Excel, PowerPoint), and visually based learning aids.

Demonstrated life-skills training, program development, and group facilitation skills (participant-to-staff ratio of up to 12:1)
-
Proven ability to work independently with minimum supervision, in classroom and/or community-based settings.

Strong crisis prevention and intervention,, communication, interpersonal, organizational, conflict resolution, and leadership skills.
-
Ability to work well under pressure to ensure minimal interruption in service delivery.
-
Direct experience engaging with diverse populations and an ability to apply a culturally responsive lens to all interactions, including program implementation and capacity-building.
-
Valid First Aid and CPR and CPI certifications; CPI certification required or willingness to obtain.
Preferred Qualifications:
Ability to speak French
