Job Description Baycrest Hospital has an opportunity for a
REGISTERED NURSE (RN) Transitional Care Unit (TCU) (3 West) Position Type: Permanent Part-Time
Shift Type: Day, alternate weekends ( subject to change) Hourly Pay Rate: $39.07 - $56.00
Bi-Weekly Hours: 15 Hours
Hours of Work: 7.5 hrs/shift

The Transitional Care unit (TCU) provides care for patients who are awaiting discharge to an alternative level of care setting and prepare them for discharge. The philosophy of the unit is to maintain health, wellness and function while awaiting placement in an appropriate client centered environment.
Responsibilities include but are not limited to:
- Utilizes sound clinical knowledge and assessment skills, works as clinical expert and resource and provides comprehensive, quality client and family centred health care services from admission, through transfer and discharge
- Plans for, delivers and evaluates nursing interventions, including medications, as well as treatments which fall within the RN’s scope of practice
- Provides client-centred care within interdisciplinary care team to meet the personal care, activation, recreation and spiritual needs of clients
- Participates actively as a preceptor or mentor for other nurses and nursing students
Qualifications include but are not limited to:
- Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required
- Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is required
- BScN
- Demonstrated ability to take professional accountability for leading, organizing, providing and delegating care activities for clients and families
- Psychogeriatric experience, i.e. mental health assessment and intervention and working with responsive behaviours preferred
- Excellent interpersonal, communication and problem solving skills
- Basic computer skills – Electronic Medical Records, Meditech, Microsoft Word and Excel preferred
