Reporting to: Director, Ottawa Health Team

Pay Scale: $48.865/hr to $57.496/hr

ry: We are seeking a System Transformation Lead to work with primary care, allied health care partners and the Ottawa OHT-ÉSO backbone team to support the planning and implementation of collaborative health and social care initiatives aimed at improving health outcomes and system integration across the Ottawa OHT-

ÉSO.

Reporting to the Director of the Ottawa OHT-ÉSO, the System Transformation Lead will work closely with partners across primary care, home and community care, community support services, hospitals, mental health substance use health and addictions and other sectors to lead and coordinate projects that advance the OHT’s strategic g

Our Beginning:

Founded by community members in 1979, we are one of about 60 non-profit community health centers across Ontario. We are run by a volunteer Board of Directors comprised of 12 community members elected at our annual meeting.

Growing With Our Community:

In 1979 we opened our doors with 4 staff. Today, we have over 347 full and part time employees serving our local community and over 400 volunteers.

Pinecrest-Queensway Community Health Centre is an innovative community based, multi-service center. We strive to meet the needs of the diverse communities we serve. We work in partnership with individuals, families and communities to achieve their full potential, paying particular attention to those who are most vulnerable and at risk.
