Job Description

POSITION SUMMARY

Reporting to the Community Paramedicine Supervisor these specialized Paramedics are responsible for the delivery of Community Paramedicine including; scheduling appointments, completing client assessment, completing home visits, providing treatment and follow-up, comprehensive documentation, participating in collaborative partnerships and communication with other community care providers and client family members, and reporting of required program measures. In dedicated model positions, shifts in regular operations may be assigned from time to time due to Base Hospital maintenance of certification requirements or operational exigencies. In Hybrid model positions, the primary role is 911 response with Community Paramedicine activities also taking place throughout the shift where possible. Both ACP’s and PCP’s are eligible for the program, with Advanced Care Paramedics eligible for dedicated Community Paramedicine positions only and Primary Care Paramedics eligible for both dedicated and hybrid positions. Positions will be awarded based on seniority, once recruitment and selection is complete.

DUTIES AND RESPONSIBILITIES

  1. Respond to Community Paramedicine Home/Virtual/phone visits as scheduled.

  2. Participate in the development of care plans in collaboration with the Community Paramedicine Supervisor and primary care practitioners.

  3. Collaborate with the client, community partners, primary care practitioners, client family and other stakeholders for the provision of in home and remote patient management and education.

  4. Identify client referral opportunities and connection with external support services where possible.

  5. Provide client and family with personalized education surrounding the client’s condition, care plan components, prevention opportunities and treatment goals.

  6. Identify client concerns or clinically challenging cases for review with the Community Paramedicine Supervisor and the client’s primary care provider.

  7. Respond to exacerbation calls from clients and provide appropriate treatment either through the Community Paramedicine skillset or if required, emergency response and transport to the hospital utilizing the traditional Paramedic skillset.

  8. Provides emergency care to ill or injured persons according to MOHLTC BLS/ALS PCS standards.

  9. If required, arrange transport to hospital either through 911 or other appropriate means.

  10. Participate in both quantitative and qualitative service quality initiatives and data collection.

  11. Self-education and knowledge expansion/maintenance in current chronic disease management, pathology and treatments.

  12. Interact with members of multidisciplinary health care teams, allied emergency care providers, the public and patients in a professional manner.

  13. Communicate with Community Paramedicine Supervisor regularly regarding program or client progress, operational challenges, equipment or IT issues or other concerns.

  14. Maintain confidentiality and privacy of information as per The Municipal Freedom of Information Protection and Privacy Act (MFIPPA), The Personal Health Information Protection Act (PHIPA), Personal Information and Electronic Documents Act (PIPEDA) and in accordance with County of Simcoe Policies and Procedures.

  15. Complete required program documentation reports including but not limited to: SOAP notes, care plan documentation, referrals, ambulance call reports, incident reports and other data collection documentation as required under the program.

  16. Work in accordance with Occupational Health and Safety legislation, and the CSPS Standard Operating Procedures.

  17. In some positions (Hybrid CP Positions) may be required to:

a. Respond to 911 calls and operate within the County of Simcoe Paramedic Services Deployment Plan as directed by the communications center (CACC).

  1. Other duties as assigned, including redeployment in emergency situations.

EDUCATION, TECHNICAL SKILLS AND CERTIFICATION

  • Hold current certificate from a recognized Community Paramedicine program, or successfully complete the County of Simcoe Paramedic Services – Community Paramedicine Education Program when offered

  • Currently certified by an Ontario Base Hospital and qualified to work as a Primary Care Paramedic, Autonomous Intravenous authorization preferred or an Advanced Care Paramedic.

  • Must maintain all qualifications and certification as Primary Care Paramedic (PCP) or Advanced Care Paramedic (ACP) in Ontario, as defined in the Ambulance Act and by CSPS.

  • May be required to work during pandemic or under outbreak conditions.

  • Be able to attend conferences, seminars, and participate in mandatory training sessions to keep abreast of changes/trends affecting areas of responsibility.

  • Have a demonstrated understanding of adult learning principles and ability to provide education to clients and families with consideration to the level of health literacy or presence of impairments that may be present among clients and families.

  • Have knowledge of and demonstrated ability to incorporate core competencies including customer service, communication, teamwork, flexibility/adaptability, initiative/self-management, accountability.

  • Strength in computer literacy utilizing MS Office applications.

  • Strong communication skills and the ability to foster a positive and cooperative working relationship with multidisciplinary health team members, patients, other agencies and the public.

  • Must be able to work flexible shift patterns.

  • Valid G class drivers license and access to reliable vehicle.

  • Criminal records check that is less than 6 months old at time of hire.

EXPERIENCE

  • Minimum three (3) years Paramedic experience.

EFFORT

  • Must maintain Paramedic certification and be able to perform the traditional duties of a Paramedic.

  • Be in excellent physical health and able to lift heavy weights, as prescribed by patient care standards.

  • Be able to work under pressure & make rapid appropriate decisions.

  • Visual attentiveness required when driving.

  • Physical effort is required in the position when responding to calls and lifting/moving patients.

  • Attention to detail when completing reports.

  • Dexterity when performing invasive procedures, mental acuity in undertaking medical decision making and treatment.

WORKING CONDITIONS

  • Exposure to inclement weather and road conditions.

  • Ability to work independently in patients’ home environments.

  • Frequent exposure to highly stressful situations when dealing with patients in crisis.

  • Exposure to potentially stressful, unpleasant/emotional situations.

  • May be required to wear uncomfortable protective equipment.
