About the Position: The successful candidate will assist and provide services for Home and Community Care under the Aboriginal Diabetes Initiative (ADI) program for LLRIB community members. You will be working in the homes of our clients in collaboration with the Diabetes Nurse Educator. You will assist with organizing and hosting community wellness events.

Duties:

  • Will be required to monitor blood sugar, provide teaching related to diabetes & order/deliver diabetes related supplies;
  • Use skills & interventions to meet physical, social/cultural, emotional & spiritual needs to the clients;
  • Apply a knowledge-based Home Health Aide practice independently & in collaboration with the client & other health care professionals to promote independent living of clients;
  • Working 5 days per week/7.5 hours per day

Qualifications:

  • Home Health Aide or Continuing Care Aide Certificate preferred, or combination of educations & experience may be considered;
  • Previous Home Care or health related field experience is an asset;
  • Must be able to work independently & collaboratively within a multi-disciplinary team;
  • Must be self-directed & demonstrate organizational skills;
  • Strong computer skills;
  • Ability to speak or understand Cree is an asset;
  • First Aid/CPR Training Certifications is an asset &/or willing to learn;
  • Must possess a Valid Class 5 Driver’s License.
