POSITION SUMMARY:

The Social Worker conducts patient-centred assessments and performs a variety of clinical interventions based on those assessment findings including (but not limited to): crisis intervention, counselling to patients & families, provision of resources, referrals and linkages to community partners. A priority for this role is the ability to work as a collaborative, supportive member of an interprofessional team within a fast-paced environment. The expectation is that the successful candidate will be fully versed in patient advocacy, current related legislation and have a broad knowledge of community resources.

RESPONSIBILITIES:

As a Social Worker, you will:

  • Demonstrate the ability to work independently and collaboratively as part of an interprofessional team
  • Possess strong critical thinking skills with the ability to set priorities and work effectively in a fast-paced environment.
  • Demonstrate excellent organization, time management skills; and excellent written and verbal communication skills
  • Possess the ability to respond to urgent emotional distress referrals of patients and/or families
  • Use your broad knowledge of community resources, services and programs, with experience in navigating challenges in access
  • Work closely developing and maintaining relationships with colleagues and community agencies to ensure an integrated service response is accessible to individuals requiring supports
  • Assessment, counselling and community integration
  • Individual, Family and Group skills
  • Familiarity with consent and capacity legislation
  • Possess the ability to work in and contribute to an evolving program requiring flexibility and adaptability; commitment to participation in continuous program improvements to maximize direct patient care across the continuum
  • Share your commitment to continuous education and supporting clinical placements for learners
  • Demonstrate reliable attendance, punctuality, availability and performance

QUALIFICATIONS:

  • Masters Degree in Social Work
  • Registered in good standing with the Ontario College of Social Workers and Social Service Workers
  • Ability to work in a fast paced environment
  • Recent hospital experience preferred
