What you will do:
- Assess nutritional status and develop individualized, culturally appropriate care plans aligned with client needs and resources.
- Provide nutrition counselling, education, and training on meal planning, food safety, and adherence to care plans.
- Monitor client progress using weight, labs, and outcomes, and adjust plans as needed.
- Collaborate with healthcare and community partners, including referrals, advocacy, and resource coordination.
- Maintain professional, ethical, and safe practice by adhering to privacy, consent, health, and safety standards.
What you will need to be successful:
- You are registered as a member in good standing with the College of Dietitians of Alberta.
- Familiarity with tube feeds would be considered an asset.
- Your focus is to embrace person and family-centered care.
- You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions.
- You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently.
- You value the role a dietitian plays in keeping clients at home when that is of personal importance.
- You have the ability to travel within community as part of the CBI Health Home Health Services team.
