The Royal Ottawa Mental Health Center and the University of Ottawa have collaborated to establish a dual-role pharmacist position comprising of 60% clinical pharmacist services and 40% academic involvement with the University of Ottawa’s new French Pharmacy program.
Duties:
- Provide pharmaceutical care across clinical programs including medication reconciliation, care planning, and interdisciplinary rounds
- Collaborate with healthcare teams to optimize patient outcomes and resolve drug-related issues
- Disseminate drug information and support evidence-based decision-making
- Serve as a preceptor for pharmacy learners (students, residents)
- Participate in drug formulary reviews and drug use evaluations.
- Participate in the development, implementation and teaching activities related to the University of Ottawa French Pharmacy Program.
- Outreach, creative activities, research and service as assigned by the University.
- Engage in hospital committee work and/or project work, policy and/or process development and/or review.
- Participate in research activities, including research dispensing.
Qualifications:
- B.Sc. in Pharmacy or Entry Level Pharm. D
- Advanced Postgraduate Degree in Clinical Pharmacy from a recognized university OR Accredited Pharmacy Residency Program Registered to practice (Part A) as a pharmacist in the province of Ontario preferred
- Registered to practice (Part A) as a pharmacist in the province of Ontario (Ontario College of Pharmacists)
- Membership with Canadian Society of Health-System Pharmacists preferred
- Five (5) years experience in a hospital clinical pharmacy position
- Teaching experience is considered an asset
- Experience or familiarity in CPOE (computer prescriber order entry), BMV (bedside medication verification, eMedRec (electronic medication reconciliation), eMAR (electronic medication administration records), EMR (electronic medical/ health record) is considered an asset
- Strong knowledge of hospital pharmacy standards (OCP, NAPRA, CSHP)
- Experience in the medication reconciliation process
- Proficiency in English and French oral expression (advance level), written (advance level) and comprehension (advance level)
Salary Range:
51.80 to 64.50 per hour
