What you would be doing:
The Client Intake Assessment Coordinator is the first point of contact for clients and caregivers, guiding them through intake, navigation, and admissions. This role conducts interRAI CHA (Community Health Assessment) assessments to support client-centered care planning, manages waitlists, and connects clients to community resources, ensuring seamless access to services and high-quality care.

The ideal candidate will possess:

  • Post-secondary degree or diploma in the human services, other health care related field, or equivalent combination of education and experience.
  • Knowledge of community resources and relevant legislation related to community support services and experience working in a unionized setting.
  • Experience using Microsoft computer software, interRAI CHA platform and other Databases for the purpose of conducting interRAI CHA assessments and applying interRAI guidelines.
  • Experience in training, mentoring, or providing oversight on assessment practices is preferred.
  • Experience in developing individualized care plans.
  • Demonstrate customer service, conflict resolution, problem-solving and effective communication skills (verbal and written, active listening).

Compensation:

  • Salary is commensurate with experience and qualifications.

Other Considerations:
This is a full-time, permanent position with a schedule that consists of Monday – Friday 8:30am – 4:30pm. Scheduled time in the Barrie office will be required as well as travel within Simcoe County. Access to a reliable vehicle and valid insurance is required. Your site office may change to a different location in Simcoe County based on operational needs.
