Opportunity:
The Seniors Community Connector roles are the heartbeat of Parkgate Society’s Social Prescribing Program within our Seniors Services department. This outreach work drives the day-to-day operations and associated programming for this service offering. Our Community Connectors receive referrals from North Shore medical sources, connect one-on-one with isolated and at-risk seniors needing support to find appropriate programs and services in their areas and then present the needs to the associated community partners.
Working closely with and as part of the community connector team, this part-time role will work both in community visiting sites and homes. They will contribute to the overall effectiveness and success of our services for seniors, helping them to feel better connected to the community so they can live life better.
Contract Details:
- We are located in beautiful North Vancouver surrounded by forests, walking trails and the ocean; Parkgate Society shares a physical space with Parkgate Community Centre, just up the hill from Deep Cove;
- This is a part-time position, working 10 hours per week, flexible dates & times, ideally split to 2-3 days per week;
- Hourly wage is $25.00
- Vacation will be paid as a % of wages on each pay

Key Responsibilities:
- Connect with medical and health professionals to receive senior participant referrals
- Interview referred seniors to determine their needs
- Source and attend seniors’ social programs to determine suitability
- Navigate and refer participants to local services and programs
- Monitor participants’ program effectiveness and adjust programming schedule as required
- Record program data and statistical information to maintain and complete reporting requirements
- Oversee volunteers’ placements, schedules and performance
- Build and maintain relationships with program participants and address any questions, suggestions, concerns, and complaints as they arise
- Assist with the departmental advertising and promotion of seniors programming
- Participate in learning activities along with other Community Connectors
- Perform other duties and responsibilities that may arise, based on the needs of the program
Other Requirements:
- A second language is considered a strong asset
- Strong working knowledge and experienced background working in a not-for-profit, particularly in a community based and/or social service organization in programming
- Specific previous experience working in or with seniors’ programming and services
- Experience and working knowledge of available community resources within other not for profit organizations in the North Shore specifically for Seniors, would be an asset
- Strong interpersonal, relationship building and communication skills
- Demonstrated teamwork, facilitation, public speaking and leadership skills
- Strong ability to adapt and pivot in a fast-paced environment
- Personal vehicle available for work, along with a current & clear driver’s license
- Demonstrated ability to work independently and as well as part of a team
- Ability to work effectively and build strong working relationships with a broad range of program staff, volunteers, not-for-profit or publicly funded groups, agencies and organizations is critical for this role
Other Requirements:
- Clear Criminal Records Check for Vulnerable Persons
- Minimum 19 years of age
- Valid Standard First Aid Certificate
- Legally eligible to work in Canada
