Job Description:
The Inpatient Palliative Care Unit at Trillium Health Partners (THP), located at the Mississauga Hospital (MH) site, consists of 16 beds dedicated to providing specialized palliative and end-of-life care to patients with life-limiting illnesses.
Working in close collaboration with palliative care physicians and an interprofessional team, the Registered Dietitian is a highly skilled, adaptable clinician with strong critical thinking and communication abilities. In this role, you will provide patient- and family-centred care focused on optimizing comfort, function, and quality of life. Care delivery emphasizes symptom management, maintenance of mobility and dignity, and support for patients and families navigating complex and sensitive stages of illness.
Recognizing the unique needs of this population, care is guided by patient preferences, functional tolerance, and evolving goals of care. Family involvement and education are integral components of care delivery.
Reporting to the Clinical Manager, the ideal candidate is committed to delivering exceptional, compassionate care in a dynamic and emotionally complex environment. This role offers the opportunity to practice with empathy, clinical excellence, and professionalism while supporting patients and families through end-of-life care and decision-making.
Key Qualifications:
- Degree in Human Nutrition and Dietetics combined with an accredited Dietetic Internship program
- Current registration with the College of Dietitians of Ontario
- Ideally possesses membership with Dietitians of Canada
- Candidates with recent clinical experience, knowledge, education, or certification in palliative care preferred
- Established clinical skills in nutritional management of medical and complex patients
- Experienced in the management of enteral and parenteral nutrition support
- Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively within a team environment
- Effective interpersonal, organizational and communication skills
- Demonstrated ability to utilize practice-based research to ensure excellence in the provision of nutritional care.
- One to two years recent experience with established clinical skills in an inpatient acute care setting
- Experience in providing education to patients and families in individual and group settings
- Experience with end of life care is preferred
- Experience with tracheostomies is preferred
- Satisfactory attendance and performance required
Job Description:
- Provide quality nutrition care in relation to the patient's diagnosis, treatment, age, and nutrition status in an inpatient setting
- Provide screening and assessment, contribute to care plan development, establish goal-oriented treatment interventions, patient/family education and provide support to address nutritional needs
- Collaboration with the inter-professional healthcare team, patients and their families to achieve patient goals and augment the treatment plan as needed
- Act as a coach/mentor/consultant using sound critical thinking skills to achieve patient outcomes.
- Act as a coach/mentor for dietetic interns and other students as appropriate
- Participate in inter professional patient care teams as required
- Complete daily workload statistics
- Providing education to the inter-professional team to support evidence-based care
