This posting

This posting will be used to fill one permanent full-time position working 75 hours bi-weekly. It will establish a 12-month eligibility list which may be used to fill similar positions.

This job is based in the thriving and diverse city of Whitehorse in the beautiful Yukon.

About us

The Yukon Home Care Program provides client-focused health services in the home, improving quality of life and supporting Yukoners to live independently. Home Care offers a range of services tailored to meet individual needs, including support for acute care, chronic and palliative conditions. These services are determined through standardized assessments to assist in creating individualized plans of care; and are supported by an interdisciplinary team, consisting of Physiotherapists, Occupational Therapists, Registered Nurses, and Home Support Workers.

This opportunity

We are looking for a flexible, creative Physiotherapist with excellent problem solving, leadership and decision-making skills, to coordinate Home Care in Yukon communities outside of Whitehorse. The role of the Community Liaison Coordinator (CLC) is to supervise home support workers in rural, remote communities, along with coordinating discharges from hospital, completing assessment for long-term care and performing case management, If you have experience supervising staff, collaborating with community partners, and case management, then this position is for you. You must be an excellent communicator, very comfortable working in a cross-cultural community setting and able to travel to communities outside of Whitehorse on a regular basis.

Essential qualifications

Please submit your resume clearly demonstrating how you meet the qualifications below. Selections for further consideration will be based solely on the information you provide in your resume.

  • Bachelor or Master’s degree in Physiotherapy

  • Experience planning, developing and implementing programs

  • Experience supervising support staff

  • Experience as a Physiotherapist providing assessments and treatments in a community environment is an asset

  • Strong ability to collaborate and work within a multi-disciplinary team to promote community development

  • Strong skills working with various cultural backgrounds

  • Excellent organizational, interpersonal and conflict management skills

  • Effective oral and written communication skills

  • Ability to work independently within in a team setting

  • Ability to supervise and provide leadership for support staff

  • Strong assessment skills

Conditions of employment

You must meet the following conditions before being hired, or within the time frame noted.

  • TB screening
  • RCMP Security Clearance with Vulnerable Sector Check
  • CPR-C with AED (within last 30 months)
  • WHMIS 2015
  • Valid Class 5 driver’s license A valid & current license to practice physiotherapy in the Yukon obtained through The Professional Licensing & Regulatory Affairs Branch (Community Services)

Job requirements

This job requires you to perform, and/or be exposed to, the following:

  • Travel within the territory

  • Exposure to pets, exposure to second-hand smoke, latex products, flowers, and/or plants and other irritants while in client homes

  • Exposure to infectious agents
