About the role

THE ROLE

As a member of the peopleCare Management Team, this position is directly responsible for the management and leadership of all RAI LTCF Assessment and outcome data, and the multidisciplinary documentation practices in the home. This person must be a currently certified RN or RPN with 3 years of related experience. You are a self-motivated individual who possesses effective communication, analytical, and assessment and leadership skills and independent decision-making abilities. High level computer competency is a requirement. The successful candidate possesses a valid Certificate of Registration from the College of Nurses of Ontario. Experience in a long term care residence is preferred.

Reports to: Director of Care

RESPONSIBILITIES

  • Supervision, monitoring and providing on-site support to all multidisciplinary RAI users in the home;
  • Responsible for providing RAI LTCF training, education and orientation to the multidisciplinary team in the home
  • Communicating regularly with the RAI users and other members of the management team;
  • Responsible for the accurate data submission to Canadian Institute of Health Information following all the responsibilities and timelines as outlined in the facility agreement
  • Directly responsible for the Compilation and the communication of the RAI LTCF schedule to the multidisciplinary team. Ensuring they use this calendar to direct the assessment and re-assessment schedule for all residents in the home
  • Supervision of RAI users to develop processes that ensures data accuracy, an efficient data collection process, assessment completion and data utilization
  • Responsible to ensure that the home’s RAI process according to peopleCare policies and procedures, MOHLTC regulations, College Standards of Practice and inter RAI requirements;
  • Ensuring MDS assessments, resident assessment protocols, and care planning are completed within the timelines as directed by the RAI LTCF manual
  • Provision of the Outcomes Scales to the Leadership of the home in the form of reports with benchmarking as per the peopleCare Quality Improvement Process
  • An active and reporting member of the home QI committee and Professional Advisory Committee
  • Responsibility for developing and maintaining data accuracy audits and the development of action plan for issues that are determined by these audits
  • Is responsible for the Nursing Restorative Program in the home, ensuring it meets all set criteria and requirements as determined by the Ministry of Health and LTC Regulations and the policy and procedures of peopleCare
  • Ensures the care of all front-line staff is conducted with a restorative approach
  • Acts as the supervisor for the PSW Nursing Restorative Champions and is responsible for determining their resident assignments, routines and is responsible for their performance appraisals and individual performance planning
  • Monitors, evaluates and reports the effectiveness of the program through routine reporting of indicators, outcome measures and resident success stories to the multidisciplinary team
  • Is responsible for keeping records, benchmarking and reporting on the various outcome scores and measures for the home to the Executive Director and leadership team with the focus on Quality improvement over time
  • Is responsible for the generation of quarterly reporting using CIHI e-reports and benchmarking as determined by the Director of Informatics and Data Quality, preparing and reporting to the leadership team, professional advisory committee and through to corporate benchmarking
  • As the documentation lead for the home, the RAI Coordinator is responsible for the routine and ongoing Data Quality Audit or all RAI users and providing feedback, education to the RAI LTCF users in the home.
  • Is responsible for development and administration of the orientation for new staff in technology documentation and RAI LTCF
  • Is responsible to report on and assist in the decision making for staffing patterns in the nursing department based on care needs determined by ADL and RUG outcome scores from the RAI LTCF assessment
  • Ensures staff competency by reviewing documentation daily, and routinely auditing for coding and accuracy and understanding through the RAP and Care Planning process
  • Responsibility for the education of nursing and other allied health staff of RAI LTCF assessments, including coding, RAP’s, RUGs, QIs and generated outcome measures
  • Will be responsible to participate in the discipline process with staff who do not comply with the documentation requirements and procedures they are responsible for in their job descriptions
  • Assumes responsibility for the operations of the home as an active member f the leadership team and a responsibility to participate in the homes “manager of call” rotation
  • Is the facility contact for CIHI submission process and software vendor
  • Is the facility contact and liaison for the software vendor
  • Is responsible to ensure the ongoing development of the electronic resident chart and to maintain the documentation practices and strategies developed by peopleCare in the home
  • Is responsible to maintain all educational requirements as outline by the Ministry of Health, interRAI and Canadian institute for Health Information
  • Is responsible to comply with individual testing and Data Accuracy reviews as determined by ministry of Health, interRAI and Canadian Institute for Health information
  • Is responsible to complete RAI LTCF Credentialing and maintain this designation through employment in this role
  • Displays an attitude that is focused on working safely and ensuring that the work environment is safe and healthy
  • Uses safe work practices
  • Reports any actual or potential hazard to the immediate supervisor
  • Uses or wears personal protective equipment or clothing as required
  • Promptly reports personal injury to supervisor and seeks first aid as needed
  • Participates in fire safety activities and knows the facility fire and disaster plan
  • Maintains a clean and orderly work area
  • Does not use or operate any equipment in a way that would endanger any worker, resident or visitors

EDUCATION AND EXPERIENCE

  • High level of computer competency
  • Experience in adult education/training of peers
  • Ability to handle/manage strict timeframes and deadlines
  • Ability to organize, plan and implement
  • Good clinical and care planning abilities
  • Intense concentration required
  • Management time constraints and scheduling
