Job Description:
In accordance with the Vision, Purpose, and Values, and strategic direction of Island Health (Vancouver Island Health Authority), patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Reporting to the Manager, the Community Service Coordinator is responsible for facilitating an integrated interdisciplinary team of health care professionals and support staff; determines workload assignments; schedules and coordinates the efficient use of staff, equipment and other resources to ensure delivery of appropriate and safe client care. The Community Service Coordinator plans, organizes, coordinates, participates in and evaluates the delivery of integrated health care services. This position assists with program development and continuous quality improvement for an assigned client care program specialty(s) such as, but not limited to, palliative care, HSCL, dementia/facility coordination, diabetes/skin, heart health, seating/young disabled/CSIL, Indigenous/delegation/transfer of function, etc.
QUALIFICATIONS:
Education, Training And Experience
Graduation from a recognized nursing program plus three (3) years recent, related experience; or an equivalent combination of education, training and experience. Knowledge of pertinent legislation, Ministry of Health policies and gerontological issues and practices. Registration with BC College of Nurses and Midwives as a practicing RN registrant, or a practicing RPN registrant.
