The Clinical Nurse Specialist (CNS) provides leadership and promotes excellence in family-centered nursing care through the five main role components of expert practice: education, consultation, research and leader/change agent. The CNS integrates nursing and medical skills and knowledge within a broad scope of advanced nursing practice and assumes a leadership role in influencing clinical nursing practice and the provision of child and family centred care. The CNS integrates research into practice, provides comprehensive health care to specific patient populations, is a member of the health care team, and serves as the contact person for select patient populations. We are currently looking for a CNS to join the Immunology and Allergy Team.
Here's What You'll Get To Do
- Provide leadership within the interprofessional team to provide comprehensive, culturally sensitive health care including assessment, management and ongoing follow-up of patients and their families who present with symptoms suggestive of underlying genetic defects of their immune system
- Integrate current knowledge of genetic, immunodeficient and metabolic conditions into care provision and consultation
- Co-ordinate and deliver specialty care, e.g., infusion therapies, and consultations within the hospital and in collaboration with clinical staff in the Medical Short Stay Day Centre
- Collaborate with nursing colleagues to ensure each patient/family has an identified primary contact should they require guidance regarding their child's care or condition
- Foster inter-professional collaboration in the development and coordination of patient plans of care
- Provide relevant health education to children and families that is based on their needs, learning styles and stage of development
- Lead the development of relevant patient care and education resources for patients/families
- Assume a leadership role in advancing the quality of clinical practice and the provision of care
- Provide mentorship and coaching to support the development of nursing and interprofessional colleagues
- Identify, integrate, evaluate and disseminate research-based innovations for improving patient care
- Collaborate to create, utilize and/or disseminate new knowledge using formal and informal channels
- Identify and establish collaborative partnerships with services and resources across the organization, as well locally, provincially and nationally as required
- Provide leadership in planning for and fostering successful transition of patients to adult services
Essential Requirements Here's What You'll Need
- Master of Nursing or equivalent with a clinical focus
- Registration with the College of Nurses of Ontario in good standing, General Class or Extended Class
- A minimum of three years of clinical nursing experience in pediatric care, experience in complex paediatric care preferred
- Experience managing aseptic interventional procedures in pediatric care preferred
- Demonstrated ability to act as a change agent, and foster collaborative teamwork
- Innovative critical thinking with excellent communication (spoken/written) and problem-solving skills
- Ability to work in a fast-paced dynamic environment
- Ability to establish effective partnership relationships that leverage strengths of others
- A commitment to understanding and aiding in the pursuit of equity, diversity & inclusion
Assets
- Experience in an Ambulatory or similar setting
- Excellence and proficiency in the utilization of healthcare electronic systems inclusive of EHR, Microsoft platform, etc.
