Reports to : Clinical Director, Patient Flow (Rehab/CCC)

Hours: 37.5 hours per week

Salary : $105,983 to $132,483 per annum

Status : Permanent Full-Time

Position Summary The Manager, Patient Flow – Discharge & Transitions, Rehab/CCC will have operational accountability for supporting development / standardization / education / implementation and improvement of patient discharge / ALC processes across UHN Rehab/CCC sites. This role ensures timely, safe, and coordinated discharges, while enhancing patient experience, system efficiency, and clinical outcomes.

The manager collaborates with interdisciplinary teams, external partners, and system stakeholders to remove barriers to flow, implement best practices, and support strategic initiatives aligned with organizational priorities.

To support patient centred discharges and transition, the Manager, Patient Flow - Discharge and Transitions will work closely with Ontario Health at Home and our Community Service providers to enable seamless transitions. Building relationships with our Community Partners and developing structures to engage them on a regular basis in transitioning patients back to the community will be a key to success.

This new role will work closely with the Clinical Directors across all of UHN’s Rehab/CCC sites to ensure a consistent, coordinated approach to patient flow & optimization of all rehab/CCC beds at UHN.

Leadership & Strategy

  • Lead the development and implementation of patient flow strategies across Rehab & CCC, with an emphasis on optimizing patient flow & bed capacity, and reducing Alternative Level of Care (ALC) across all UHN Rehab/CCC sites.
  • Collaborate with clinical leaders to align flow initiatives with organizational priorities (e.g. Home First Philosophy), quality, safety, and patient-centered care goals.

Operational Management

  • Monitor and manage daily patient flow operations, ensuring timely transitions and discharge planning across all Rehab/CCC sites.
  • Identify and address bottlenecks, delays, and inefficiencies in post-acute care access and discharge planning.
  • Oversee escalation processes and coordinate with bed management and access teams.
  • Facilitate weekly complex discharge rounds in collaboration with Clinical Directors
  • Employ metrics & key performance indicators to continuously evaluate and improve patient flow.

Team & Stakeholder Engagement

  • Lead and support a team of flow coordinators, discharge planners, and allied health professionals to support patient flow & transition planning.
  • Facilitate interdisciplinary collaboration across acute and post-acute settings.
  • Build strong relationships with external partners (e.g. Ontario Health at Home, long-term care, acute care partners).

Performance & Quality Improvement

  • Use data and analytics to monitor flow metrics, identify trends, and drive continuous improvement.
  • Lead or support quality improvement initiatives related to transitions of care.
  • Ensure compliance with regulatory standards and organizational policies.

Working Conditions

  • Full-time position, primarily onsite with some flexibility for remote collaboration.
  • Primary location is West Park; will require regular travel across UHN Rehab/CCC sites, as well as occasionally to partner sites or regional meetings.

Qualifications

  • Bachelor’s degree in a health-related field required.
  • Master’s degree in Health Administration, Nursing, or related discipline preferred.
  • Minimum 4 years of progressive leadership experience in healthcare, with a focus on patient flow, discharge planning, or post-acute care coordination.
  • Strong leadership and change management skills
  • Excellent communication and stakeholder engagement abilities
  • Knowledge of healthcare systems, post-acute care models, and community resources
  • Proficiency in data analysis and performance improvement methodologies
  • Ability to work in a fast-paced, complex environment with competing priorities
