We are looking to hire two Interprofessional Clinical Leads.
The Thames Valley Family Health Team is one of the largest Family Health Teams (FHT) in Ontario. With more than 125 physicians across 21 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive, collaborative primary health care.
Thames Valley Family Health Team (TVFHT) is committed to excellence in leadership where Clinical and Administrative (Operations) leaders collaborate to achieve its Mission. The Interprofessional Clinical Lead provides supervisory and management support to Interprofessional team members providing Team Based Primary Care Services for communities served by TVFHT. The Interprofessional Clinical Lead reports to the Director, Clinical Services
REQUIREMENTS:
- Regulated Health Professional, registered and in good standing with Ontario Regulated Health Professional College
- Five years experience in a specified regulated health profession and primary care interprofessional team leadership preferred
- Commitment to Living the TVFHT Values as described in the Leadership Team Agreement
SKILLS & ABILITIES:
- Understands and supports performance optimizing scope of practice within the interprofessional primary health care team
- Collaborates to lead and support implementation of the TVFHT Strategic Plan and corresponding priorities
- Builds active supportive relationships with Interprofessional Healthcare Providers assigned for supervision
- Provides leadership for the provision of population-based primary health care including social determinants of health
- Appreciates the opportunity to synthesize issues of professional practice and legislation to enable service delivery
- Champion for promising and evidence-based practices as the foundation for quality improvement
- Constantly committed to earning trust, prioritizing a culture of collaboration and generosity
- Assists interdisciplinary team members with role clarification and achieving optimal scope of practice
- Collaborates with internal and external partners to foster innovation for the development and implementation of primary care core services and care pathways
- Drives individual and team performance with constant focus on Patient Experience and Outcomes in a performance managed culture
- Effectively communicate and supervise performance (QI, KPI, Monitoring, Evaluation) for a multidisciplinary team
- Communicates effectively, especially where there is need for crucial conversations
- Decision maker, problem solver, change manager and committed to conflict resolution
- Reviews Directives in conjunction with Medical Director and Clinical Director
- Proficient skills in Microsoft Office applications, specifically: Word, PowerPoint, Excel, and Outlook, as well as Internet research skills
- Hybrid working model that involves virtual work plus travelling to different sites to provide on site support
Salary: $95,848 - $116,889 (dependent on years of experience)
