Functional Recovery Support Case Manager

As a Functional Recovery Support Case Manager, you will manage a caseload of individuals engaged in treatment or post-assessment services, supporting them across all stages of recovery — from assessment and engagement through to sustained recovery and return to work (RTW). You’ll work as part of a collaborative team to deliver tailored recovery planning, connect clients to needed supports, and ensure progress is monitored and communicated effectively.

What You’ll Be Doing
- Manage a service caseload implementing and reviewing ongoing treatment plans while liaising closely with the full range of services, workers, and referral sources.
- Connecting participants to needed community resources and recovery supports, including but not limited to health care, SUD treatment, and mental health treatment, with an array tailored to the participant's needs and goals
- Work with the participant, the referral source and other healthcare providers to support the participant
- Provide updates to Clinical Lead and referral source with regards to compliance with treatment recommendations
- Work collaboratively with service providers to promote individual service user progression from engagement, through structured treatment.
- Monitor each case to ensure the participant is attending as required on the pre-approved timetable and intervention count
- Review the content of Service Provider documentation and identify the need for clinical lead involvement if there are concerns about the content
- Document activity within clinical records in accordance with departmental expectations
What We’re Looking For
- Bachelor’s degree in Psychology, Social Work, or related health discipline preferred (equivalent experience considered)
- 2–5 years of experience in administration or case management, with preference for case management background
- Knowledge of case management principles and practices is an asset
- Ability to read and interpret policy wording
- Strong organizational, decision-making, and communication skills
- Ability to work independently while contributing effectively to a team
- Strong problem-solving, conflict management, and attention to detail
- Caring, approachable, objective, and committed to exceptional client service
- Proficiency with MS Office (Word, Excel, PowerPoint)
