HOURS OF WORK:

The stated hours of work are approximate as schedules are subject to change with the operational needs of the Health Centre. Changes made to hours of work will follow the terms of the applicable collective agreement, company policy and/or legislation.

WHAT YOU’LL DO:

The RN, as one of the providers of health care, has a unique role in promoting wellness and health, in preventing illness and responsive behaviours, and in helping patients attain and maintain the highest level of health possible. S/he is responsible for providing comprehensive care to patients, with predictable and unpredictable outcomes who may or may not be clinically stable. The RN practices in accordance with the College of Nurses of Ontario Professional Standards.

WHAT YOU’LL BRING:

· Current Certificate of Competence from the College of Nurses of Ontario.

· BCLS Required

· B.Sc.N. required

· Demonstrated extensive knowledge working with individuals with various types of dementia, delirium, mental health addictions, and other neurological conditions, and those who experience an acute behaviour change

· Enhanced knowledge of family and caregiver needs who can be best served by a behavioural support system

· Training/knowledge of Gentle Persuasive Approach (GPA), P.I.E.C.E.S. TM, U-First! Recovery/psychosocial rehabilitation approaches or willingness to receive training

· Experience/familiarity with RAI documentation including assessments, Resident Assessment Protocols (RAPS) and care planning functions

· Glucometer certification required

· Evacuation training

· Current clinical practice in Geriatrics, LTC and or Mental Health would be an asset

· Courses/Certificates in Gerontology, Behavioural Management, Mental Health, Conflict Resolution and/or Neurology preferred

· Previous experience or willing to train for MDS (RAI) & RUGS

WHO YOU ARE:

· Added Nursing Skills - Venipuncture, IV initiation, medication, chronic vent training, CVADs, insert, set-up and maintain G-tubes, insert catheters and performs manual irrigations, initiates and maintains CADD and Hickman lines

· Demonstrated person and caregiver focused care and supported by best evidence based clinical practices

· Good understanding of non-pharmacological and therapeutic behavioural approaches

· Good understanding of pharmacological agents, and their interactions, that are used to manage conditions that have behavioural components along the disease continuum

· Demonstrated trouble shooting and problem solving approach relative to behavioural exacerbations

· Excellent assessment skills

· Demonstrated ability in planning and coordination of care and resources for residents/patients

· Excellent verbal communication and documentation skills

· Good critical thinking skills for the early detection, intervention and prevention of responsive behaviours

· Good conflict resolution and de-escalation skills

· Excellent interpersonal skills and ability to work effectively with patients/families, staff, physicians and the community

· Demonstrated enthusiasm, flexibility and ability to cope with change in previous/present work experience

· Commitment to change management

· Ability to demonstrate leadership skills including facilitation, coaching, mentoring and adult learning skills

· Demonstrated effectiveness working with minimal supervision with proven reliability and trustworthiness

· Demonstrated ability to work toward the achievement of goals and objectives, and to initiate appropriate revisions when necessary

· Demonstrates good planning and organizational skills

· Demonstrated effectiveness as a team member through collaboration, respect and effective communication skills in interacting with patients, family members and other members of the inter-professional care teams including community and primary care team members

· Demonstrated commitment to continuing education

· Good computer skills - EPIC, RUGS, Intranet

· Proven good attendance record, organizing and prioritizing work effectively and emergency management effectiveness
