About the role

POSITION SUMMARY

John Howard Society of Kingston & District has been selected as the agency responsible for the administration of case management for the cabin initiative – a development which has transitional housing and intensive services for individuals experiencing homelessness.

This position will be responsible for the case management duties for the residents of the cabins, as well as the coordination of collaborative service delivery with community partners.  We are seeking a highly motivated individual to fill the role as Case Worker - Cabin Initiative. This individual will report to the Team Lead - Cabin Initiative on a fixed term contract position, 37.5 hours/week, Monday to Friday, for a 1 year contract. The annual salaruy for this positon is $53,656. 

Duties and Responsibilities

Resident Management

  • Ensure all intake documentation is completed, and filed
  • Maintain all client files in accordance with internal policies 
  • Ensure new residents receive orientation information and supplies
  • Ensure that all residents comply with the rules of residency (in conjunction with case manager and support staff) 
  • In collaboration with Program Manager: 
    • Coordination of issuance of tiered infractions 
    • Ensure eviction and trespassing orders are carried out according to policies and procedures
    • Oversee prompt resolution of vacancies and coordination of new intakes
  • Advocate for clients. Promote the development of more effective services through new initiatives or service improvements/enhancements

Programming and Service Delivery

  • Facilitate programming with other staff in collaboration with community partner agencies 
  • Ensure clients are provided with programming that addresses concerns including but not limited to: 
    • Social/recreation connections 
    • Arts
    • Identification
    • Employment/Education 
    • Mental health
    • Substance Use
    • Health/Nutrition
  • Assess client programs for effectiveness and appropriateness

Administration

  • Ensure client file notes are maintained in the client information management system
  • Maintain client master files by affixing assessment and intervention plan, statistical forms, consents, and any relevant internal information, including all housing program forms

SKILLS AND ABILITIES

  • Organizational  and interpersonal skills
  • Excellent written and verbal communication skills
  • Respect for professional boundaries and confidentiality
  • Ability to function as a member of a team
  • Computer skills, e.g. Microsoft Office, internet, email
  • Able to motivate and give direction to residents 
  • Flexibility
  • Ability to represent the agency in public forums

EDUCATION AND EXPERIENCE

  • Post-secondary education in humanities and/or social services
  • Demonstrated experience in case management, residential housing programs, homelessness, mental health
  • Knowledge of concurrent disorders and harm reduction approaches
  • Knowledge of motivational, solution-focused and crisis intervention counselling techniques
  • Extensive knowledge of community resources
