Wage range: $22.60 - $28.25 per hour
Hours: 37.5 per week
Shifts: 8 - 12 hour shifts - Days, Evenings and/or weekends
Status: Permanent Full-Time. Contingent on continued funding.

Job Summary:
Reporting to UHN@Home, the Home Dialysis PSW will assist patients undergoing home dialysis by delivering direct care and ensuring the patient’s comfort and safety throughout the dialysis treatment. Additionally, the Home Dialysis PSW will support other duties as needed for non-dialysis patients, including general patient care tasks and other responsibilities as required.
Duties:

Performing home dialysis for patients based on the delegation from a nurse or other regulated health care provider

Providing direct patient care and ensuring the comfort and safety of the patient during dialysis sessions.

Working with an external vendor to manage the supplies and inventory in the patient’s home
-
Maintaining documentation pre, intra and post dialysis to understand the patient’s tolerance to the treatment to support program and operational reporting needs
-
Assisting non-dialysis patients with activities of daily living such as grooming, dressing, and bathing, meal planning and preparation, mobility and transfers and light household duties such as cleaning, laundry, and washing dishes

Includes but is not strictly limited to performing basic patient care functions, which contribute to the implementation of the plan of care.

Provides care using their knowledge, skills and abilities, including how to report outcomes and escalate safety concerns.
Qualifications
At minimum, completion of high school and 1-year Personal Support Worker Certificate from a recognized college/Occupational Therapist Assistant Certificate or Physiotherapy Assistant Certificate (Ontario College Certificate) from an accredited community college or a recent UHN approved private career college
- Completion of a Basic Cardiac Life Support (BCLS) program is required
- 2+ years of practical experience
- Experience with direct patient care is required
- Ability to handle daily mild to moderate physical exertion (ex: constant standing, walking, climbing, reaching, sitting)
- Computer literacy and basic proficiency is required with a demonstrated willingness to learn and adapt to new technology
- Fluency in English (verbal and written) is required
- Access to a reliable vehicle and the ability to travel to patient locations as required is mandatory
- A valid driver’s license, adequate insurance (personal automobile policy with a minimum 1$ million (liability limit)
