About The Job Social Workers perform comprehensive assessments, care planning, and treatment coordination for clients and their families. Collaborating with a multidisciplinary team, you'll provide psychosocial care, counseling, system navigation, and advocacy, while connecting clients to specialized resources and supporting their self-management. Your role focuses on improving the health and social well-being of the identified population.
