Location: KidsInclusive - Portsmouth site, 221 Portsmouth Ave. (primary), HDH, Brockville, community visits in KFLA, LLG and HPE

DESCRIPTION:

As part of the Southeast Region Infant Hearing Program (IHP), the Social Worker will provide skilled social work services to the clients of KidsInclusive (KI) and their families including individual and family systems models. The social worker works with the child and family to assess their strengths, needs, supports and goals. Services may be provided in the home, centre, daycare, school, community or virtually.

Each employee must demonstrate an awareness of, and be responsible for, supporting and providing client and family-centered care. Employees will demonstrate dignity and respect by being courteous and introducing themselves by name, occupation and role. Wherever possible, the social worker will ensure that clients and families receive complete and accurate information and have the opportunity to ask questions. They will encourage clients and families to participate in care and decision-making at the level they choose, and collaborate by involving the client and family in the formulation of a plan of care.

PRINCIPLE DUTIES & RESPONSIBILITIES:

Clinical:

  • Assist the family as they begin to adjust to their child’s confirmation of Permanent Hearing Loss, and if appropriate, provide supportive counseling;

  • Support the family’s understanding of the information provided by the IHP Audiologist, including the child’s hearing level and its impact on language development, and refer back to the IHP Audiologist for clarification, as appropriate;

  • Help families understand the services provided under the IHP;

  • Work collaboratively with the IHP Audiologist to share information and ensure consistent messaging is shared with families about the child’s hearing level and intervention strategies;

  • Support a family’s decision-making related to their child’s language development (e.g., facilitating connections with other parents of children who are Deaf/Hard of Hearing, if available);

  • Provide linkages to other professionals and organizations, based on the family’s needs and goals for their child;

  • Support the family in accessing resources (e.g., exploring funding options, locating child care);

  • Coordinate Communication Development Plan (CDP) meetings (if identified by the team as the person to do so), in collaboration with other team members;

  • Participate in CDP meetings;

  • Work collaboratively with service providers (within and outside the IHP, for example with the Service Planning Coordinator for Coordinated Service Planning) to develop a coordinated plan of support for the child and family that reflects an understanding of the family’s unique situation (e.g., emotional state, finances, language, etc.); and

  • Support families during transitions, in collaboration with other team members (e.g., to child care, to school).

Administrative:

  • Maintain a record of all daily activities in AlayaCare as per KidsInclusive policy

  • Ensure maintenance of all client information, in accordance with privacy legislation and KHSC/KidsInclusive policies.

  • Participate in team meetings, discipline meetings and KidsInclusive meetings as well as regional meetings of the Ontario Infant Hearing Program

  • Participate in fieldwork supervision and evaluation of social work student placements

  • Provide mentoring and shadowing opportunities as requested by management

  • Provide input to the development of department and or hospital quality improvement procedures as requested.

  • Assist in maintaining inventory of equipment and supplies; make recommendations for purchases.

Educational:

  • Maintain a standard of professional education by attending educational programs and workshops; transmit information obtained from these workshops to fellow employees.

  • Complete e-learning modules as required by KHSC/KidsInclusive policies

  • Assume educational responsibilities as required by KidsInclusive

  • Maintain a record of all educational activities on AlayaCare

Each employee is accountable for:

MINIMUM QUALIFICATIONS:

  • Current membership in the Ontario College of Social Workers and Social Service Workers.

  • Bachelor of Social Work (BSW) with 2 years’ experience in related health or community based services for children with complex needs and their families;

  • Demonstrated experience and initiative in family and/or group counselling, particularly for families with children with complex needs;

  • Demonstrated clinical management and communication/interpersonal skills including working as a collaborative team member within own discipline and on multi-disciplinary team(s).

  • Continuing education courses related to practice

  • Access to a reliable vehicle along with a valid driver's license and automobile insurance as per hospital policy

  • Proven ability to attend work regularly.

  • Satisfactory criminal reference check and vulnerable sector search required.

DESIRABLE QUALIFICATIONS:

  • Proficiency in speaking and comprehending English, French and American Sign Language.

  • Prior experience working with individuals with permanent hearing loss and in the areas of Coordinated Service Planning and Healthy Child Development programs.

PHYSICAL REQUIREMENTS:

The incumbent will be required to meet the physical demands as described for the area of work.
