Job Description:

POSITION PURPOSE SUMMARY

The Registered Practical Nurse (RPN) provides safe, competent, and person-centered nursing care within the scope of RPN practice under the direction of the Director of Care and the Registered Nurse. The role contributes to the broader mission and values of The Salvation Army, upholding standards of service that reflect its spiritual and social commitment to the community.

ACCOUNTABILITIES / RESPONSIBILITIES

  • Incumbents may perform all, some, or a similar range of duties as outlined below:
  • Deliver safe, effective, and person-centered nursing care in accordance with the RPN scope of practice, individualized care plans, and regulatory standards.
  • Administer medications and treatments accurately in accordance with physician orders and medication administration protocols. Monitor resident responses and report changes in condition promptly.
  • Maintain complete, accurate, and timely clinical documentation in PointClickCare (PCC) including progress notes, assessments, care plans, medication administration records, and incident reports.
  • Participate in resident assessment and support the planning, implementation, and evaluation of individualized care plans in collaboration with residents, families, and the interdisciplinary care team.
  • Provide wound care management, including wound assessment, dressing changes, monitoring healing progress, and communicating concerns to the wound care lead or physician.
  • Participate in Resident Safety Incident (RSI) management, including incident reporting, documentation, monitoring residents after incidents, and implementing follow-up interventions.
  • Observe and report changes in residents’ physical, emotional, cognitive, or behavioural status to the Registered Nurse, Nurse Practitioner, or physician as appropriate.
  • Communicate effectively with residents and family members, providing updates regarding care, responding to concerns, and ensuring respectful and compassionate interactions.
  • Provide direction and clinical support to Personal Support Workers (PSWs) to ensure safe and effective delivery of resident care.
  • Promote and maintain infection prevention and control (IPAC) practices, including outbreak management protocols and symptom surveillance.
  • Support quality improvement initiatives, clinical audits, and regulatory compliance requirements.
  • May act as shift lead when assigned by nursing leadership, coordinating workflow, resident care needs, and staff assignments.
  • Represent the organization in a respectful and professional manner, reflecting the mission and values of The Salvation Army.
  • Perform other related duties as assigned.

CRITICAL RELATIONSHIP MANAGEMENT

Internal:
Ministry Unit leadership, nursing staff, interdisciplinary team members, and volunteers.

External:
External agencies (e.g., EMS, health care service providers), residents and their family members, and students.

MANAGERIAL RESPONSIBILITIES

Reports directly to the Director of Care.

WORKING CONDITIONS

  • Work is primarily performed indoors within the long-term care facility in a fast-paced clinical environment.
  • The role requires sitting, standing, walking, lifting, and computer/keyboard use.
  • Shift work including evenings, nights, weekends, and statutory holidays may be required.
  • Some scheduled overtime may be required.
  • The incumbent may occasionally interact with individuals displaying challenging behaviours.

EDUCATION AND EXPERIENCE QUALIFICATIONS

Education, Qualifications and Certifications

  • Graduate of an approved Practical Nursing Program.
  • Current registration and in good standing with the College of Nurses of Ontario.
  • Current certification in First Aid and CPR.

Experience and Knowledge Requirements

  • Minimum three years of related nursing experience preferred.
  • Knowledge of nursing standards, clinical protocols, and long-term care practices.
  • Experience using electronic health record systems such as PointClickCare is considered an asset.
  • Basic proficiency in Microsoft Office applications.

Compensation:

The target hiring range for this position is $30.98 to $32.99 with a maximum of $35.00.

Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
