About the role

Beacon Community Services is seeking a compassionate, communityminded individual to support older adults in improving their wellness and staying connected to their communities. Using a social prescribing approach, you will work alongside seniors to understand what matters most to them and connect them with supports that enhance their quality of life. As the Community Connector, you will meet oneonone with older adults to cocreate personalized wellness plans, provide practical guidance, and help them access programs such as exercise groups, meal supports, and housing resources. Through trustbuilding, outreach, and clear documentation, you will help reduce isolation, build confidence, and strengthen meaningful connections to community.

There are two positions available.

  • Temporary full time - Monday to Friday 37.5 hours per week until the end of September 2026
  • Regular part time - 18.75 hours per week

KEY RESPONSIBILITIES

Client Support & Navigation

  • Receive, screen, and triage referrals for the Social Prescribing program
  • Complete intake and strengthsbased assessments, including frailty/functional screenings
  • Cocreate individualized wellness plans based on client goals, cultural needs, and interests
  • Provide shortterm case management including regular followups, problemsolving, and encouragement
  • Assist seniors in accessing community programs, preventative health supports, and practical resources
  • Conduct home and community visits and accompany clients to initial programs when needed
  • Develop plainlanguage materials to help seniors understand available services

Community Connections & Outreach

  • Build and maintain strong relationships with primary care providers, health authorities, and seniorsserving agencies
  • Represent Beacon/SHOAL at community tables, outreach events, Healthy Aging collaboratives, and Communities of Practice
  • Support volunteers involved in program delivery (friendly visits, transportation, activities)
  • Respond to general inquiries (including Better at Home) and direct clients to the correct regional provider

Program Development & Reporting

  • Maintain and update an asset map of community programs, culturally specific supports, and local activities
  • Input and maintain accurate records in the database and Excel tracker
  • Complete funder reporting (including quarterly/annual reports for United Way BC)
  • Analyze data and client feedback to identify service trends and recommend improvements
  • Safeguard personal information in alignment with PIPA and organizational policy

Qualifications

  • Postsecondary education in Community Social Services, Gerontology, Social Work, Health Sciences, or related field
  • 2–5 years’ experience supporting older adults or vulnerable populations
  • Experience with intake screening, assessments, wellness planning, and shortterm case management
  • Knowledge of seniors’ services and community resources
  • Comfortable presenting information to community and health care partners
  • Valid Class 5 BC Driver’s Licence and reliable transportation for community/home visits

Skills & Abilities

  • Strong relationshipbuilding, listening, and communication skills
  • Ability to work respectfully with older adults from diverse cultural backgrounds using traumainformed and culturally safe practices
  • Organized and able to manage a caseload, documentation, and deadlines
  • Skilled in Microsoft 365 (Word, Excel, PowerPoint, Teams) and database systems
  • Comfortable working independently in community settings (including client homes)
  • Able to safely perform duties that may involve walking, standing, and transporting materials
