Job Summary And Requirements This posting This posting will be used to fill 1 permanent full-time position working 75 hours biweekly. It will establish a 12-month eligibility list which may also be used to fill future permanent and term positions.

This job is based in the thriving and diverse city of Whitehorse in the beautiful Yukon .

Who We Are The Yukon Healthy Living Program (YHLP), formerly known as the Chronic Conditions Support Program, is a collaborative care team that supports Yukoners living with chronic health conditions. Support is provided in the form of one-on-one and group programs and services that are accessible throughout the territory. YHLP has expanded its service capacity through various forms of virtual care including the use of zoom for health and Cloud DX connected health kits for Home Health Monitoring. Members of our team collaborate in a clinical setting to empower clients and families to self-manage chronic conditions while also supporting our program goals through the provision of high quality, collaborative chronic care, and effective and accessible self-management support.

Your role As a Chronic Conditions Nurse Educator, you will form an integral part of our team by providing quality, planned, proactive, person-centered chronic disease management (CDM) within the Yukon Healthy Living Program (YHLP). CDM activities include client assessments, determining chronic conditions risk factors, clinical care, education (one-on-one & group), self-management support, as well as care-giver support and education. You will collaborate as a member of the YHLP team in a clinical setting to empower clients and families to self-manage chronic conditions while also supporting our program goals through the provision of high quality, collaborative chronic care, and effective and accessible self-management support.

Important Application And Job Requirements Essential qualifications Please submit your resume clearly demonstrating how you meet the qualifications below. Selections for further consideration will be based solely on the information you provide in your resume.

  • Registered nurse in good standing with a Canadian registered nursing regulatory body;
  • Demonstrated applied knowledge of chronic disease management through case management of clients with chronic conditions;
  • Experience facilitating learning of others through workshops, training, or teaching;
  • Experience performing point of care (POC) services such as venipuncture, foot checks, foot care, spirometry, ABIs, ECGs, and any other relevant Chronic Disease Management POCs would be considered an asset.
  • Certified Respiratory Educator and/or Certified Diabetes Educator would be considered an asset.

  • Strong knowledge and experience in nursing best practices and standards.
  • Ability to work autonomously and also within a multi-disciplinary team environment.
  • Excellent oral and written communication, analytical, problem solving and organizational skills.
  • Excellent ability to initiate relationships with clients, their families and other health professionals.
  • Ability to provide direction and deliver services in a competent, safe, and ethical manner.
  • Strong engagement, facilitation, and presentation skills; and,
  • Proficient in the use of computers and software including MS Teams, Word, Excel, and PowerPoint.

Conditions of employment You must meet the following conditions before being hired, or within the time frame noted.

  • Current registration with Yukon Registered Nurses Association (YRNA);
  • RCMP Vulnerable Sector Security Clearance;
  • TB Screening;
  • Valid class 5 driver’s licence or equivalent;
  • Valid CPR-HCP or equivalent.
  • WHMIS 2015 (within one month of hire)

Job Requirements This job requires you to perform, and/or be exposed to, the following:

  • Willingness to travel by road and aircraft.
  • Overnight travel is expected as part of this position to provide service to clients in our communities.
  • Lifting up to 15kg which may include audio visual and clinical supplies and equipment.
  • May be required to assist patients with ambulation
  • Regularly exposed to blood/infectious agents, bodily fluid, scents and/or needle punctures that may carry pathogens
  • Exposure to emotionally distressed patients and families in isolated clinical settings
