As part of Insight Health Solutions, The THP Solutions Clinic is an innovation initiative delivering virtual care to the community. The program currently delivers physician to physician support through the Seamless Care Optimizing Patient Experience (SCOPE) program; medical assessment and treatment in collaboration with the Community Paramedicine Program, virtual specialist care to nursing homes, and to patients directly. The program is launching a pilot clinic in October 2025 to further scale and test the virtual care platform to offer patients with lower acuity issues to access an assessment with an NP in a HealthPod. These self-contained private and secure HealthPods are equipped with biometric devices and virtual care-enabling technologies that connect patients with healthcare providers for high-quality, remote medical assessment and treatment.

Position Summary Reporting to the Program Manager at Insight Health Solutions, the Nurse Practitioner will utilize their comprehensive theoretical knowledge base and advanced level of clinical competence to support the development and clinical needs of exciting innovative programs at Insight Health Solutions.

This Nurse Practitioner will act as a clinical resource and lead the clinical design, implementation, and operational support for upcoming clinical initiatives requiring Nurse Practitioner expertise, such as but not limited to, the Solutions Clinic and its programs and provide clinical expertise for development of clinical protocols, patient flow processes, and monitoring quality indicators to ensure safe, effective, and patient-centered care.

This Nurse Practitioner will also provide coverage as required in Insight Health Solutions for programs which include but are not limited to the Infusion Clinic, IHS' Occupational Health services for employers, in-person medical assessment and` treatment services to employers, and patients directly

The Nurse Practitioner (NP) will utilize a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for patients within these programs. This will involve performing patient care assessments and coordinating care for patients with complex co-morbidities, ordering appropriate diagnostic tests, pain and symptom management, and patient education. The NP will be instrumental in helping patients navigate the system to improve access and optimize safe and high-quality outcomes thereby, enhancing a positive patient experience.

As a nursing leader, the Nurse Practitioner will perform quality improvement, monitoring and evaluation, complete administrative activities and participate in the development/revisions of policy and procedure. Other key components of the role may entail supporting research activities; developing, delivering, coordinating and participating in education and professional development activities/programs; performing activities to further develop and strengthen the profession; acting as a resource and mentor; and serving as a consultant to individuals and groups within the professional community and other hospitals/agencies.

Responsibilities: ( The NP works within 4 domains of practice) Clinical Domain

  • Demonstrates the NP competencies in providing primary health care services to patients presenting to the Solutions Clinic.
  • Conducts medical assessments and follow-up assessments and formulates a differential diagnosis through the integration of patient information and evidence-informed practice.
  • Must be able to provide clinical care both virtually and in-person.
  • The NP works autonomously to diagnose order and interpret diagnostic tests, prescribe pharmacotherapy and make referrals to other health care professionals.
  • Counsels' patients, substitute decision makers and families on medication therapy, benefits, potential side effects and possible interactions.
  • Supports the interprofessional team to provide clinical care for, including making independent decisions for patient treatment and discharge, including completion of discharge summaries and transfer of accountability to community care providers.
  • Supports the operational readiness of the clinic by monitoring supply levels and identifying when replenishment may be required. Collaborates with clinic staff to ensure appropriate inventory is maintained to support safe and efficient patient care.
  • Provides support for the clinical design, implementation, and operational support for any upcoming clinical initiatives, including assessing patients, developing individualized care plans, coordinating multidisciplinary input, and supporting the day-to-day operations to ensure clinic readiness and smooth workflow.
  • Participates in quality improvement initiatives and contribute to protocol development and refinement.
  • Conducts initial medical assessments and arranges follow up care as required.
  • Works collaboratively with the interdisciplinary team.
  • Liaises with family physicians, community nurse practitioners and other medical specialists to connect patients to community resources.
  • Refers for diagnostic imaging, laboratory tests and other specialist care as required.
  • Documents clinical data, assessment findings, diagnoses, plans of care, interventions, patient responses, and clinical rationale in a timely and accurate manner.
  • Supports the development of clinic policy and practice standards to support the patient experience in partnership with Physician and administrative leadership.

Knowledge Translation

  • Participates in the development and evaluation of the clinical programs including collecting and documenting data as required for program evaluation.
  • Analyzes, appraises and synthesizes research and best practices to use as a foundation for enhancing the delivery of quality patient care.
  • Uses knowledge translation techniques in innovative ways to put research-based knowledge into daily practice and to support and educate other members of the health care team.
  • Contributes to the dissemination of nursing knowledge through presentations and publications.

Scholarship

  • Evaluates outcomes of practice, practice patterns and systems of care against local and national benchmarks to identify variances in practice outcomes and population trends.
  • Engage in evidence-informed practice by critically appraising and applying relevant research, best practice guidelines and theory when providing healthcare services.
  • Identifies issues related to clinical practice and patient care and develops innovation ways to improve care and care delivery for patients, families, and communities.
  • Designs, directs, participates in and evaluates quality improvement methodologies to promote safe, timely, effective, efficient, equitable, and patient-centered care.
  • Participates in continued learning opportunities.

Leadership

  • The NP acts as a transformational leader by using ideals, inspiration and intellectual stimulation to influence the behaviours and attitudes of others towards the goal of excellent patient care.
  • Acts as a consultant and resource to members of health care team and external groups in the process of decision-making strategic planning and problem solving by committee involvement, participation in program development and evaluation.
  • Participates in scheduled program and individual meetings to support and shape program development and quality improvement efforts.

Key Qualifications Minimum masters level nurse practitioner preparation

  • Primary Health Care NP (Registered Nurse in the Extended Class) in good standing with the College of Nurses of Ontario Master’s degree in Nursing with specialized clinical focus in Nurse Practitioner Practice.
  • Completion of education to prescribe controlled substances required by the College of Nurses of Ontario or equivalent.
  • Advanced assessment and diagnostic reasoning skills
  • Excellent communication skills and the ability to develop positive relationships with patients, families, physicians and inter-professional team members
  • Experience with research methodology, program planning and evaluation
  • Current Basic Cardiac Life Support certification
  • Satisfactory performance and attendance required
  • Demonstrated experience and leadership in education and research.
  • Current experience and demonstrated success as a Nurse Practitioner Practice in primary care in a using a virtual platform.
  • Demonstrated and comprehensive understanding of symptom management related to this patient population is required.
  • Ability to function as an independent practitioner and a collaborative team member.
  • Membership in the Nurse Practitioners’ Association of Ontario (NPAO) or Registered Nurses’ Association of Ontario is strongly preferred.
  • Excellent team building, negotiation and conflict resolution skills.
