About the role:

The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)

is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values

and ensures that those values guide all decisions. Creating a caring environment for patients and families ensures

continuity of care and fosters a learning environment. Applicants are required to have a demonstrated knowledge,

understanding and commitment to this care philosophy.

Within Prevention and Screening Clinical Service, the Preventive Care Program offers participants an opportunity to

have a conversation(s) about chronic disease prevention and holistic health with a trained prevention specialist(s) to

co‑develop a personalized preventive care plan.

Reporting to the Manager, Prevention and Screening Clinical Services, the Prevention Specialist works with

participants to co‑develop personalized preventive care plans. The Prevention Specialist will utilize health

assessment tools, education and coaching to support participants address a variety of health and wellbeing topics. A

key deliverable will be working with participants around goal setting and preventative care planning along with

creating connections to community resources to aid participants. The Prevention Specialist will work independently

and also work with the mobile cancer screening. The Prevention Specialist will be guided by Ontario

Health‑Preventive Care Program as well as TBRHSC policies and procedures.

EMPLOYMENT REQUIREMENTS:

Education/Experience:

* Minimum 2 year community college diploma

* Degree or diploma in a health related field (i.e. Bachelor of Science, Practical Nursing, Medical Laboratory

Technology, etc.).

* Current unencumbered license or registration from applicable regulatory body (i.e. College of Nurses of Ontario,

College of Medical Laboratory Technologists) if applicable

* 3 years healthcare related experience

Skills/Abilities:

* Ability to navigate a variety of electronic health information and client management systems

* Skills in communication, conflict resolution, and critical thinking within a health care team

* Highly organized individual with strong initiative who enjoys a flexible work environment

* Strong time and priority management

* Committed to patient and family centred care while working professionally and in a positive manner with a diverse

team of health care professionals

* Knowledge of community based health resources, regional health care, primary care provider and hospital

operations

* Proficiency in MS Office (Excel, Word, PowerPoint), Visio

* Valid G Driver's License with access to a vehicle

Assets:

* Previous community health experience an asset

This position will require travel to multiple locations for clinics, mobile clinics, and outreach. Attendance at occasional

outreach events for the department may be during the evening or weekend.

Salary Range: $36.730 to 45.910
