Your Opportunity:

Forensic Assessment & Community Services (FACS) has an exciting opportunity for a Single Independent Living Support (ILS) Worker. Reporting to the Care Manager of FACS, this position will act as a member of multidisciplinary teams that provide treatment services that are evidence and recovery based, aligned with best practice approaches, policy, legislation, and designated standards of practice within the context of an outpatient, forensic community mental health clinic. The successful candidate will be responsible for supporting clients, under the supervision of a registered health professional, with navigating social determinants of health, accessing resources, completing various applications, and supporting with transition from hospital to community when appropriate. The successful candidate will collaborate with colleagues to support client treatment plans in the areas of independent living. These tasks may include bus training, budgeting, life skills, support with attending appointments and connecting with the client in the community. This position works closely and collaboratively with community stakeholders, supporting with case management planning and follow-up. This will include but is not limited to clients' with developmental delays, acute mental health issues, and complex social needs.

Description:

As a Single Independent Living Support (ILS) worker, you will be part of an interprofessional team, assisting patients (client, resident) to develop and maintain skills as identified in the care plan, associated with living independently in the community. You will provide support with preventative health care, health maintenance, quality of life, life skills coaching and connect patients to appropriate community supports. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of Recovery Alberta.

Required Qualifications:

Completion of Grade 12 or equivalent.

Additional Required Qualifications:

BLS required. NVCI or immediate registration into the next available course is required.

Preferred Qualifications:

Familiarity with electronic medical records (Connect Care), virtual therapy platforms (Zoom), and comfort with Microsoft Office programs will all be considered assets.
