Reports to: Director, Project Management Office & Executive Lead, FLA Ontario Health Team

We’re seeking a Project Manager to lead and coordinate initiatives that improve primary care access and patient attachment across the Frontenac, Lennox & Addington region. This role is central to implementing Health Home attachment pathways, ensuring that every individual is connected to a Person-Centred Health Home.

PRIMARY FUNCTIONS:

• Lead project planning and implementation of the Primary Care Attachment strategy using best practices in project management.

• Engage and collaborate with a wide range of partners including primary care teams, physicians, community and specialty care providers, and patients.

• Develop and manage project plans, timelines, deliverables, and evaluation metrics.

• Support project teams by preparing meeting materials, proposals, reports, and communications.

• Coordinate evaluation efforts, including defining key metrics and compiling data into reports and presentations.

OVERVIEW OF RESPONSIBILITIES:

Project Development

• Collaborate with stakeholders to define project goals, scope, and deliverables.
• Develop project charters, master plans, and supporting documentation.
• Prepare and present reports, proposals, and updates to sponsors and executives.
• Advise on IT solutions and contribute to business case development.
• Monitor project priorities, timelines, and performance metrics.

Project Management

• Lead project execution to ensure alignment with objectives, timelines, and resources.
• Manage scope changes and update plans and budgets accordingly.
• Facilitate stakeholder engagement throughout the project lifecycle.

Risk and Issue Management

• Identify and resolve project issues to improve outcomes.
• Assess risks and recommend mitigation strategies.
• Provide guidance on business implications of risk.

Resource and Vendor Management

• Lead project teams and coordinate external vendors.
• Support procurement processes and manage contracts.
• Oversee project budgets and recommend resource allocations.

Organizational Leadership

• Contribute to development of project management tools, templates, and training materials.
• Mentor staff on project management practices and methodologies.

QUALIFICATIONS:

• University degree or equivalent.

• 3–5 years of project management experience.

• Project Management Professional (PMP) certification or equivalent.

• Strong knowledge of Project Management Body of Knowledge (PMBOK) and full project lifecycle.

• Familiarity with Ontario health system and policies (asset).

• Proficiency in Microsoft 365 and project management tools.

• Experience in budgeting, procurement, and resource planning.

• Excellent communication, stakeholder engagement, and leadership skills.

• Strong problem-solving, critical thinking, and change management abilities.

• Ability to manage multiple priorities under pressure.

• Satisfactory criminal reference and vulnerable sector check.

PHYSICAL REQUIREMENTS:

The applicant must be able to meet the physical demands of this position.
