About the role

Youth and Fami ly Support Practitioner

Permanent, 1.0 FTE Hours: Rotating Schedule. 7am-3pm 2 weeks 3-11pm 2 weeks. Sunday - Thursday rotation

The Program – Passages

The Passages Program is a six bed Live-In Program for youth ages 13 to 17 years who have status with Children’s Services. Journey’s offers a recreation, life skills and relationship-based program, with an average stay of 6-12 months. The intention of the youth’s stay is to collaborate with the youth and their supports to identify strengths and areas of growth where resourcing and skill development can be done with the aim of supporting the youth’s successful transition back to their family (parental or kinship setting) wherever possible, or into a less intrusive placement where indicated by the case plan.

The programming at Passages focuses on cultivating each youth’s sense of self-worth and helps each youth identify, establish, and maintain healthy relationships with their supports to allow for successful and stable transitions. The Passages Program provides youth with a safe and nurturing environment; individualized care plans; educational and recreational activities; and the development of daily routines. The development of strong positive relationships is a key component of the overall success of the Passages Program.

The PositionYouth and Family Support Practitioner

The Youth and Family Support Practitioner provides supervision and support to young people residing in the program and is a member of the team ensuring treatment plans are implemented. All workers are expected to assist with daily household routines and chores, minor facility maintenance and direct youth support. Youth and Family Support Practitioners must have good working knowledge of computer programs and skilled with basic office applications as there are daily administrative tasks as well as case management tasks as part of this role.

Education and Experience

To be considered for this opportunity, applicants should have minimally a degree/diploma in the Human Service field, experience with youth in a group living environment; be aware of trauma informed practices, and be professional, reliable and able to work independently.

Other Qualifications

  • Current Standard First Aid and CPR C

  • Current Criminal Record Check with a Vulnerable Sectors Designation

  • Valid Class 5 driver’s license (Class 5 GDL will not be accepted) with a clear driving record.

Principle Based Practice Enviros’ Principle Based Practice is a model that utilizes principles to guide and drive action in practice, a decision-making tool in our interventions addressing critical areas of need, and the foundational structure of support for the crucial work we do incorporating strengths and culture into our day-to-day interactions. This model utilizes core principles to ensure a holistic approach is taken when supporting people. It ensures there is direction from the person served and an integrated planning mechanism where natural supports are present.
