Job Summary

This position functions as part of the Home’s leadership team and is responsible for the management of all aspects of resident care, including developing and implementing quality programs and services to meet the needs of clients in a manner consistent with the standards of long term care.

Duties & Responsibilities

  • Responsible for the ongoing development of the teams and for providing supervision, leadership, coaching and direction to the nursing and personal care staff of the home in the performance of their duties.
  • Act as a role model and contribute to the development of a caring, inclusive, and competent team who is responsible and accountable for direct resident care activities, as outlined in the resident’s plan of care and the Regions’ Policies and Procedures.
  • Assists and supports the planning, development and implementation of quality resident care programs in accordance with the Fixing Long Term Care Act and Regulations, the College of Nurses of Ontario standards, interRAI LTCF, and Services for Seniors Mission, Vision, and Philosophy of Care.
  • Supports the management of all aspects of resident care in an empathetic, efficient, and effective manner.
  • Supports the development of a resident centered environment with a strong focus on a high level of resident satisfaction and quality of life.
  • Develops and maintains positive, proactive partnerships with families to ensure satisfaction with care and services.
  • Analyses and evaluates resident care and services to drive improvements.
  • Continuously supports the developments, implementations, and evaluation of quality improvement/best practice initiatives participates in the Continuous Quality Improvement Program, including ongoing participation in the Services for Senior’s accreditation process.
  • Supports the ongoing education of staff, including onboarding and annual mandatory training.
  • Takes proactive measures to supports the team to ensure regular attendance through consistent application of the Attendance Management Program and collaboration with scheduling services.
  • Conducts performance evaluations and performance management as required.
  • Ensures strong fiscal accountability by contributing to the administration of the resident care budget.
  • Works with all staff to ensure consistency and service excellence within the home and among the Villages of Halton Homes.
  • Participates in Health and Safety activities and as a member of the Home’s Joint Health and Safety Committee as required.
  • Performs other duties as assigned.

Skills & Qualifications

  • Currently registered with the College of Nurses of Ontario.
  • Diploma in Nursing, Bachelor of Nursing degree or working towards.
  • Long Term Care experience at the nursing management level with a solid knowledge and understanding of applicable legislation, including the Fixing Long Term Care Act, and InterRAI-LTCF.
  • Proven experience in leading, supporting, and coaching teams to achieve quality resident centred care outcomes.
  • Experience in the development, implementation and evaluation of programs to support quality care and leading staff.
  • Advanced knowledge of MS Word, Excel, PowerPoint, and Outlook.
  • Proficient knowledge of Point Click Care.

Working/ Employment Conditions

Working Condition

  • A flexible work schedule is required to respond to the needs of the home, requirement to work every third weekend.

  • Travel will be required; incumbent must provide their own transportation.

Employment Condition

  • Current (obtained within the past six (6) months), original and acceptable Criminal Records Check with Vulnerable Sector Screen, by the first day of employment.
  • Current (obtained within the past six (6) months) two-step tuberculosis test or Quanti-FERON-TB Gold plus test by the first day of employment.
