Nurse Clinician, Diabetes
About the role
Sinai Health is looking for a Nurse Clinician for the Leadership Sinai Centre for Diabetes (LSCD). The role is primarily located at our Mount Sinai Hospital campus. The Nurse Clinician is a Certified Diabetes Educator (CDE) nurse who will provide leadership within LSCD. Emphasis will be on care of patients and their caregivers with type 1 diabetes, complex type 2 diabetes and/or diabetes in pregnancy. The LSCD has built a strong reputation as a diabetes centre which serves patients with complex needs requiring diabetes management in both the inpatient and outpatient settings. In collaboration with the interdisciplinary team at the LSCD, the Nurse Clinician is responsible for assessing, developing, implementing and evaluating the effectiveness of innovative clinical and educational services for patients and their caregivers.
About This Team More than 20,000 patients visit our diabetes and endocrinology clinics at the Leadership Sinai Centre for Diabetes (LSCD) each year. Our centre is a national leader in providing care for people with complex diabetes, thyroid disease and other endocrine conditions. We also specialize in caring for pregnant people with pre-existing Type 1 and Type 2 Diabetes, as well as those with gestational diabetes mellitus (GDM). Our program is recognized for advancing diabetes care and research locally, provincially, nationally and internationally.
In This Role You Will
- Work with the interdisciplinary team to provide clinical care with a focus on education and disease management to patients and caregivers
- Demonstrate a comprehensive theoretical knowledge base and advanced level of clinical competence in the care of patients with diabetes
- Develop and implement education curricula and materials for patients, their family and health care providers
- Collaborate on the establishment of goals and objectives for the population consistent with the provision of effective, efficient, high quality patient and family centered care
- Engage in hospital and clinic quality and performance initiatives
- Support collaborative design, implementation and evaluation of information technologies in both clinic operations and clinical diabetes management
- Support research activities and promoting evidence based clinical practice
- Perform other duties consistent with the job classification as required
Job Requirements Mandatory
- Graduate of a CNO-approved Baccalaureate Nursing (RN) program. Equivalent combined education and experience will be considered
- Currently registered and in good standing with the College of Nurses of Ontario (CNO)
- 5 years of experience in diabetes nursing
- Certified Diabetes Educator
Preferred
- Experience with Accuro
Skills and Knowledge
- Skills in diabetes management, counselling and patient self-care are key competencies
- Ability to utilize information technology
- Effective verbal and written communication skills, interpersonal and customer services skills
- Excellent organization and time management skills
- Demonstrated decision making, problem recognition, problem solving skills, negotiation and conflict resolution skills
- Commitment to collaborative practice
- Ability to work effectively independently and under high pressure conditions
- Demonstrated satisfactory work performance and attendance history
