Job Description: The Social Worker BSW will work with the Continuing Care team
to create an environment that protects the financial security and quality of life of residents. The Social Worker BSW will collaborate with Continuing Care and Home Care teams and families, clients/residents to establish a plan to meet the unmet needs for any issues and/or concerns of the resident/client/family in the areas of financial security and advanced care planning ethical decision making to identify, assess and if possible resolve the concerns or problems for clients/residents of Continuing Care facilities, as expressed by the referral source by collaborating with the family, client and/or health care team. These concerns would focus on their financial needs and address social determinants of health. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

- Bachelor of Social Work (BSW)
Licenses
- Valid Class 5 driver's license

- Demonstrated ability to use sound clinical judgement
